Application of medication reconciliation in the management of medication discrepancy in ischemic stroke patients during the hospital-home transition period
Objective:To Explore the efficacy of a medication reconciliation intervention program in the management of medication discrepancy in ischemic stroke patients in the hospital-home transition period.Methods:Patients with ischemic stroke in the department of neurology in a tertiary grade A hospital in Auhui province were selected as the study object,45 patients admitted to the hospital in July 2023 were selected as the control group,and 45 patients admitted to the hospital in November of the same year were selected as as the observation group,the drug reorganization program based on the routine nursing of the control group was implemented.The occurrence of medication deviation,medication compliance,rational drug use self-efficacy and unplanned readmission rate were compared between the two groups.Results:At 2 weeks,31 days,and 8 weeks of discharge,the incidence of medication discrepancy was lower in the observation group than that in the control group(P<0.05);medication adherence and rational medication self-efficacy were significantly higher in the observation group than that in the control group after the intervention(P<0.05).The 31-day unplanned readmission rate was lower in the observation group than that in the control group(P<0.05).Conclusion:Medication reconciliation program can effectively reduce the occurrence of medication discrepancy in ischemic stroke patients during the hospital-home transition period,improve medication adherence,and ensure patient medication safety.
