The correlation of vitamin D with maternal-fetal interface and peripheral blood inflammatory cytokines in patients with unexplained recurrent abortion
Objective To investigate the correlation between serum vitamin D[25-(OH)D]and maternal-fetal interface,peripheral blood interferon(IFN)-γ,interleukin(IL)-4,IL-17,transforming growth factor(TGF)-βin patients with unexplained recurrent spontaneous abortion(URSA).Methods From September 2021 to May 2022,30 patients with URSA admitted to Shanghai Changning District Maternal and Child Health Hospital were selected as the observation objects(URSA group),another 30 normal early pregnant women requiring artificial abortion at the same time were selected as the control group,the levels of IFN-γ,IL-4,IL-17 and TGF-β in serum and decidual tissue were detected by ELISA,the level of 25-(OH)D was detected by chemiluminescence method.Pearson method was used to analyze the correlation between serum 25-(OH)D and IFN-γ,IL-4,IL-17,TGF-β in URSA patients.Results The levels of IFN-γ and IL-17 in decidua in URSA group were(11.3±2.6)pg/ml and(20.3±3.6)pg/ml,respectively,which were higher than those in control group(4.6±0.9)pg/ml and(12.4± 2.2)pg/ml(P<0.05),the levels of IL-4 and TGF-β were(9.6±2.8)pg/ml and(15.6±3.5)pg/ml,respectively,which were lower than those in the control group(15.5±3.2)pg/ml and(24.2±4.2)pg/ml(P<0.05).The levels of serum IFN-γ and IL-17 in URSA group were(18.9±3.6)pg/ml and(41±7)pg/ml,respectively,which were higher than those in control group(12.6±2.6)pg/ml and(28±6)pg/ml(P<0.05).The levels of IL-4,TGF-β and 25-(OH)D were(25±5)pg/ml,(45±6)pg/ml and(27±5)ng/ml,respectively,which were lower than those in the control group(33±6)pg/ml,(52±8)pg/ml and(45±7)ng/ml(P<0.05).The level of serum 25-(OH)D in URSA patients was negatively correlated with the levels of IFN-γ and IL-17 in decidual tissue and serum,and positively correlated with the levels of IFN-γ and IL-17 in decidual tissue and serum(P<0.05).After treatment with vitamin D,the levels of serum IFN-γ and IL-17 in URSA group were(14.2±2.8)pg/ml and(33.7±5.0)pg/ml,respectively,which were lower than those before treatment(18.9±3.6)pg/ml and(41.2±6.8)pg/ml(P<0.05),the levels of IL-4 and TGF-β were(28±4)pg/ml and(49±5)pg/ml,respectively,which were higher than those before treatment(25±5)pg/ml and(45±6)pg/ml(P<0.05).Conclusion The levels of IFN-γ and IL-17 in URSA patients are elevated and the levels of IL-4 and TGF-β are declined.They are related to 25-(OH)D deficiency,and 25-(OH)D supplementation can regulate the expression of IFN-γ,IL-4,IL-17,and TGF-β.
