Value of serum PCT/ALB and CRP/ALB in the assessment and proanosis of craniocerebral trauma
Objective To study the value of serum procalcitonin to albumin ratio(PCT/ALB)and C-reactive protein to albumin ratio(CRP/ALB)in the assessment and prognosis of craniocerebral trauma(TBI).Methods One hundred and fifty-eight patients with TBI admitted to our hospital from January 2021 to December 2021 were selected and divided into mild(n=68),moderate(n=61),and severe(n=29)groups according to their Glasgow Coma(GCS)scores,and into good prognosis(n=110)and poor prognosis(n=48)groups according to their Glasgow Prognosis(GOS)scores.The serum PCT/ALB and CRP/ALB levels of patients in each group were compared,and Pearson correlation analysis was used to correlate the PCT/ALB ratio and CRP/ALB ratio with the GCS score.The multi-factor Logistic regression was used to analyze the factors influencing the prognosis prediction of patients with TBI.The value of serum PCT/ALB and CRP/ALB in the assessment of TBI disease and prognosis prediction was assessed using subject operating characteristic(ROC)curves.Results The levels of PCT/ALB ratio and CRP/ALB ratio were significantly higher in the intermediate and severe disease groups than those in the mild disease group(P<0.05).The levels of PCT/ALB ratio and CRP/ALB ratio were significantly higher in the severe disease group than those in the intermediate disease group(P<0.05).The levels of PCT/ALB ratio and CRP/ALB ratio were significantly higher in the poor prognosis group than those in the good prognosis group,and the GOS score was significantly lower than that in the good prognosis group(P<0.05).Pearson correlation analysis showed that PCT/ALB ratio and CRP/ALB ratio were significantly negatively correlated with the GCS score(r=-0.561,-0.548,both P<0.05),and both were significantly negatively correlated with GOS scores(r=-0.485,-0.612,both P<0.05).Multifactorial Logistic regression analysis showed that the PCT/ALB ratio and CRP/ALB ratio were risk factors for poor prognosis in patients with TBI(P<0.05).ROC curve analysis showed that the area under the curve(AUC)for PCT/ALB ratio,CRP/ALB ratio,and both combined to assess the extent of disease in patients with TBI were 0.826,0.801,and 0.879,respectively.The AUCs of PCT/ALB ratio,CRP/ALB ratio,and both combined to assess the prognosis of TBI patients were 0.795,0.815,and 0.861,respectively.Conclusion Serum PCT/ALB and CRP/ALB showed significantly high expression in patients with severe and poor prognosis TBI and were significantly negatively correlated with GCS score and GOS score,and they have some value in disease assessment and prognosis prediction,among which the combination of the two is of higher value.
