Relationship between serum HIF-1α IL-4 and severity and prognosis in patients with bronchiectasis complicated with pulmonary infection
Objective To analyze the relationship between serum hypoxia-inducing factor 1α(HIF-1α)and interleukin-4(IL-4)and the severity and prognosis of patients with bronchiectasis complicated with pulmonary infection.Methods A total of 200 patients with bronchiectasis admitted to the hospital from April 2019 to May 2021 were selected and divided into an infected group(n=122)and an uninfected group(n=78)according to whether they were complicated with lung infection.The expression levels of HIF-1α and IL-4 in the two groups were compared and then divided into mild,moderate,and severe infections according to the severity of the infected group.The expression levels of HIF-1α and IL-4 in patients with different degrees of infection were analyzed,and the patients were divided into good prognosis and poor prognosis according to the prognosis of the infection group.The clinical data of patients with bronchiectasis complicated with pulmonary infection with different prognosis were compared,and the risk factors affecting the poor prognosis of patients with bronchiectasis complicated with pulmonary infection were analyzed by Logistic regression.The receiver operating characteristic(ROC)was used to analyze the prognostic value of serum HIF-1α and IL-4 in patients with bronchiectasis complicated with pulmonary infection.Results The levels of HIF-1α and IL-4 in the infected group were higher than those in the uninfected group(P<0.05).The levels of HIF-1α and IL-4 in patients with bronchiectasis with different degrees of infection showed that mild infection group<moderate infection group<severe infection group(P<0.05).There were significant differences in FEV1,mMRC score,HIF-1α,and IL-4 in patients with bronchiectasis complicated with pulmonary infection(P<0.05).Logistic regression analysis showed that the high expression of HIF-1α and IL-4 were the risk factors for poor prognosis of bronchiectasis complicated with pul-monary infection(P<0.05).ROC curve analysis showed that the AUC of serum HIF-1α and IL-4 to predict the poor prognosis of patients with bronchiectasis complicated with pulmonary infection was 0.795 and 0.823,respec-tively,and the AUC of combined IL-4+HIF-1α detection was as high as 0.911,which was significantly higher than that of each single detection(P<0.05).Conclusion Serum HIF-1α and IL-4 can effectively evaluate and predict the severity and prognosis of patients with bronchiectasis complicated with pulmonary infection.The combined detection of the two indicators has higher clinical practical value and has certain auxiliary significance for he pathogenesis of bronchiectasis.
