Clinical Analysis of Transumbilical Single-Port Laparoscopic Extracorporeal Ovarian Cystectomy for Benign Giant Ovarian Cysts
Objective:To explore the safety and feasibility of exfoliation of benign giant ovarian cysts by umbili-cal single-port laparoscopy.Methods:A retrospective analysis was conducted on the clinical data of 72 patients with benign giant ovarian cysts treated with umbilical single-port laparoscopy in the gynecology department of the Third People's Hospital of Chengdu from October 2021 to September 2022.Among them,31 patients were in the single-port laparoscopy extracorporeal ovarian cystectomy group and 41 patients were in the single-port laparos-copy intracorporal ovarian cystectomy group.The surgical time,intraoperative pneumoperitoneum usage time,intr-aoperative bleeding volume,intraoperative cyst rupture rate,and postoperative anal exhaust recovery time were compared and observed between the two groups pain level within 24 hours after surgery,length of hospital stay.Results:The intraoperative leakage rate of cystic fluid was significantly lower in the single-port laparoscopy extra-corporeal ovarian cystectomy group(0 vs.21.1%)than in the single-port laparoscopy internal ovarian cystectomy group(P<0.05).The surgical time(60.19±5.64 min vs.72.02±6.89 min),intraoperative pneumoperitoneum usage time(15.23±2.69 min vs.53.83±6.74 min),and postoperative anal exhaust recovery time(28.16±3.53 h vs.32.24±3.87 h)of the single-port laparoscopy extracorporeal ovarian cystectomy group were all shor-ter than those of the single-port laparoscopy intracorporal ovarian cystectomy group(P<0.05).There was no difference in intraoperative bleeding volume,postoperative 24-hour VAS score,and postoperative hospital stay be-tween the two groups(P>0.05).Conclusions:In the huge ovarian cyst excision operation,the single-port lapa-roscopy extracorporeal ovarian cystectomy can greatly avoid intraoperative cystic fluid leakage in the abdominal cavity,and follow the principle of non-tumor surgery.At the same time,it can shorten the operation time and pro-mote the rapid recovery after the operation,which is safe and feasible and worthy of clinical promotion.
