Analysis of factors associated with lymph node metastasis in endometrial cancer
Objective To study the clinical,examination,imaging and pathological factors of patients with endometrial cancer(EC)before operation,and to explore its value in predicting EC lymph node metastasis.Methods The clinical data of 498 patients who were pathologically diagnosed as EC after diagnostic curettage and underwent systematic staging surgery were retrospectively analyzed.The possible risk factors related to EC lymph node metastasis were analyzed,independent risk factors were screened out,and a nomogram model was established.The predictive performance of the nomogram model was evaluated by drawing the subject operation characteristic curve.Results Among 498 EC patients,43(8.6%)had pelvic and/or para-aortic lymph node metastases.Multivariate Logistic regression analysis showed that:Carbohydrate antigen 125(CA125)>29.715 U/ml,combined diabetes,magnetic resonance(MR)suggested that lymph node enlargement,non-endometrioid adenocarcarcinoma,and total granulocyte/total lymphocyte count(NLR)were independent risk factors for lymph node metastasis in EC patients.The combined area under the ROC curve(AUC)of these five indicators predicted lymph node metastasis was 0.86,the sensitivity was 86%,and the specificity was 71.9%.Conclusion CA125>29.715U/ml,Combined diabetes,MR suggested lymph node enlargement,non-endometrioid adenocarcinoma,and NLR combined prediction showed good predictive efficacy in lymph node metastasis in endometrial cancer patients,and the LNM prediction model constructed with good sensitivity and specificity.
