Analysis of the association between HPV associated cervical adenocarcinoma Silva typing and clinicopathological features and prognosis
Objective To analyze the clinicopathological features,prognosis and Silva typing of HPV-associated cervical adenocarcinoma.Methods The clinical data of 83 patients with invasive cervical adenocarcinoma were retrospectively analyzed.The Silva classification,pathological morphology,follow-up results and pathological characteristics of each type of patients were observed,and the correlation between Silva classification and pathological characteristics and prognosis was analyzed.Results Among the 83 patients,25 were Silva A type,21 were Silva B type and 37 were Silva C type.Pathologic results:there were 21 cases of micropapillae,the incidence of Silva C type>Silva B type>Silva A type(P<0.05).The incidence of Silva C type>Silva B type>Silva A type was 45 cases(P<0.05).In 15 cases,the incidence of Silva C type was higher than that of Silva A type and Silva B type(P<0.05).The 3-year mortality rate of 83 patients was 3.61%(3/83),all of which died after recurrence.The recurrence rate was 10.84%(9/83),including 2 patients with recurrence and metastasis.The 3-year progression-free survival rate was 89.16%(74/83).Progression-free survival curve of Silva A type was better than that of Silva C type(P<0.05).There was no significant difference in progression-free survival curve between Silva A type and Silva B type(P>0.05).There was no significant difference in progression-free survival curve between Silva B type and Silva C type(P>0.05).Kapkn-Meier analysis showed that Silva classification and metastasis could affect the prognosis of cervical cancer patients(P<0.05).Conclusion There are significant differences in clinicopathological features and prognosis among HPV-associated cervical adenocarcinoma patients with different Silva typing.Silva typing can provide objective reference for clinical treatment and prognosis judgment.
