Finkler超声评分联合HE4在老年卵巢恶性肿瘤诊断及化疗疗效评估中的价值

摘要
目的 探讨Finkler超声评分联合人附睾蛋白4(HE4)在老年卵巢恶性肿瘤诊断及化疗疗效评估中的价值.方法 选取本院60例老年卵巢肿块患者作为研究对象,对其进行Finkler超声评分、HE4检测,以病理诊断结果为金标准,比较两种方法的诊断效能.并根据首次化疗结果,将卵巢恶性肿瘤患者分为无反应组和化疗反应组,比较两组的Finkler超声评分、HE4浓度.结果 以病理检查结果为金标准,最终确诊48例恶性肿瘤、12例良性肿瘤;Finkler超声评分、HE4检测和联合检测诊断卵巢恶性肿瘤的AUC值分别为0.82、0.87、0.90,联合检测的敏感度、特异度高达91.67％和83.33％;化疗后,有反应组的Finkler超声评分和HE4浓度均显著低于无反应组(P＜0.05).结论 Finkler超声评分联合HE4在老年卵巢恶性肿瘤诊断和化疗疗效评估中显示出较高价值,这种综合评估方法能够更准确地判断肿瘤性质和治疗反应,为老年卵巢恶性肿瘤的个体化治疗提供重要信息.
Abstract
Objective To explore the value of Finkler ultrasound score combined with human epididymal protein 4(HE4)in the diagnosis and chemotherapy efficacy evaluation of elderly ovarian malignancies.Methods 60 elderly patients with ovarian masses in our hospital were selected as the research subjects,and Finkler ultrasound score and HE4 detection were performed on them.The pathological diagnosis results were used as the gold standard to compare the diagnostic efficacy of the two methods.According to the results of the first chemotherapy,ovarian cancer patients were divided into a non responsive group and a chemotherapy responsive group,and the Finkler ultrasound score and HE4 concentration of the two groups were compared.Results Based on the pathological examination results as the gold standard,48 cases of malignant tumors and 12 cases of benign tumors were ultimately diagnosed;The AUC values of Finkler ultrasound score,HE4 detection,and combined detection for diagnosing ovarian malignant tumors were 0.82,0.87,and 0.90,respectively.The sensitivity and specificity of combined detection were as high as 91.67％and 83.33％,respectively;After chemotherapy,the Finkler ultrasound score and HE4 concentration in the responsive group were significantly lower than those in the non responsive group(P＜0.05).Conclusion The combination of Finkler ultrasound score and HE4 has shown high value in the diagnosis and chemotherapy efficacy evaluation of elderly ovarian malignancies.This comprehensive evaluation method can more accurately determine the nature of the tumor and treatment response,providing important information for personalized treatment of elderly ovarian malignancies.
