子宫宫腔病变应用经阴道三维超声的诊断价值探讨

摘要
目的 研究经阴道三维超声诊断子宫宫腔病变的临床价值.方法 选取88例疑似子宫宫腔病变患者,对所有患者进行经阴道三维超声诊断,以最终的确诊结果作为金标准,评价经阴道三维超声在诊断子宫宫腔病变中的价值.结果 在所有患者中最终确诊为子宫宫腔病变患者50例,而经阴道三维超声检出52例,对比差异无统计学意义(P>0.05).经阴道三维超声诊断子宫宫腔病变患者的子宫内膜容积明显比非子宫宫腔病变患者的更小(P<0.05).绘制ROC曲线,经计算可得经阴道三维超声诊断子宫内膜息肉内膜容积的AUC为0.893(95%CI 0.825,0.962),诊断宫腔粘连内膜容积的AUC为0.946(95%CI 0.905,0.987),诊断子宫肌瘤内膜容积的AUC为0.902(95%CI 0.836,0.968).结论 将经阴道三维超声应用在子宫宫腔病变患者诊断中具有较高的检出率,值得临床应用.
Abstract
Objective To study the clinical value of transvaginal three-dimensional ultrasound in the diagnosis of uterine cavity lesions.Methods 88 patients with suspected uterine lesions were selected and all patients were diagnosed by transvaginal three-dimensional ultrasound.The final diagnosis result was used as the gold standard to evaluate the value of transvaginal three-dimensional ultrasound in the diagnosis of uterine lesions.Results Among all the patients,50 cases were diagnosed with uterine cavity lesions,while 52 cases were detected by transvaginal three-dimensional ultrasonography,with no statistical significance(P>0.05).The endometrial volume of patients with uterine lesions diagnosed by transvaginal three-dimensional ultrasound was significantly smaller than that of patients with non-uterine lesions(P<0.05).The ROC curve was drawn,and the AUC for the diagnosis of endometrial polyps by transvaginal three-dimensional ultrasound was 0.893(95%CI 0.825,0.962),and the AUC for the diagnosis of endometrial adhesions was 0.946(95%CI 0.905,0.987).The AUC for diagnosis of endometrial volume of uterine fibroids was 0.902(95%CI 0.836,0.968).Conclusion The application of transvaginal three-dimensional ultrasound in the diagnosis of uterine cavity lesions has a high detection rate and is worthy of clinical application.
