摘要
目的 评价3.0T氢质子磁共振波谱(proton magnetic resonance spectroscopy,1H-MRS)在诊断肝脏肿瘤性疾病中的价值.方法回顾性分析28例受试者的3.0T 1H-MRS数据和常规MRI诊断结果.依据病理结果将波谱数据分为肝细胞癌(hepatocellular carcinoma,HCC)组,肝内转移瘤(metastatic tumor,MT)组和正常肝组织对照组.记录胆碱(Choline,Cho)峰、糖原和葡萄糖复合物(glycogen and glucose complex,Glyu)峰、谷氨酰胺和谷氨酸复合物(glutamate and glutamine complex,Glx)峰与脂质(Lipid,Lip)峰的数值,并比较3组间Cho/Lip、Glyu/Lip、Glx/Lip的值.通过受试者特性曲线(ROC curve)评价1H-MRS对肝脏肿瘤的鉴别能力.结果 HCC组Cho/Lip为0.24±0.14,MT组Cho/Lip为0.09±0.08,对照组Cho/Lip为0.09±0.05,HCC组的Cho/Lip显著高于MT组与对照组(P<0.05).HCC组与MT组的ROC曲线提示当Cho/Lip界值取0.08时曲线下面积最大,此时敏感度和特异度分别为91%和60%.结论 3.0T 1H-MRS在HCC与结肠癌肝转移瘤的鉴别诊断中具有一定价值.
Abstract
Objective To evaluate the value of proton magnetic resonance spectroscopy (1H-MRS) in diagnosing liver neoplasm using 3. 0-T MR scanner. Methods We retrospectively analyzed the 1H-MRS and conventional MRI data in 28 subjects. Based on the pathological results, all spectral data were divided into hepatocellular carcinoma (HCC) group, metastatic tumor (MT) group and a control group of normal liver tissue. The values of choline (Cho), glycogen glucose complex ( Glyu), glutamate glutamine complex (Glx) and lipid (Lip), and the ratios of Cho/Lip, Glyu/Lip, and Glx/Lip were recorded and compared among three groups. By using ROC curve, we assessed the value of 1H-MRS to identify some liver tumors. Results Cho/Lip ratios in the HCC group, metastasis one and control one were 0.24±0. 14, 0.09±0.08, 0.09±0.05, respectively. Cho/Lip ratio in the HCC group was significantly higher than that in other groups (P<0.05). As for the ratios of Glx/Lip and Glyu/Lip, there was no significant statistical difference among three groups. When the threshold of Cho in the ROC curve was 0. 08, the maximum area under the curve was seen, and the sensitivity and specificity of curve were 91 % and 60%, respectively. Conclusion 3. 0T 1H-MRS has a certain value in differentiating HCC and liver metastases of colon cancer.
