目的 探讨眼动脉超声血流参数诊断2型糖尿病(T2DM)合并视网膜病变及对病变程度的评估价值.方法 采取回顾性研究,选择2023年3月至2024年3月收治的80例T2DM合并视网膜病变患者为研究对象(病变组),另选取同期接受治疗的单纯T2DM患者为非病变组.统计并比较2组眼动脉超声血流参数[视网膜中央动脉(CRA)的收缩期峰值流速(PSV)、舒张末期血流速度(EDV)及阻力指数(RI)和眼动脉(OA)的PSV、EDV及RI];采用受试者工作特征曲线(ROC)分析眼动脉超声血流参数对T2DM患者发生视网膜病变的诊断价值.根据T2DM患者合并视网膜病变程度分组,将增生性视网膜病变患者纳入增生组(37/80),将非增生视网膜病变患者纳入非增生组(43/80);采用ROC分析眼动脉超声血流参数对T2DM患者视网膜病变程度的评估价值.结果 病变组CRA-PSV、CRA-EDV、OA-PSV及OA-EDV低于非病变组(P<0.05),CRA-RI及OA-RI高于非病变组(P<0.05);绘制ROC曲线结果显示,CRA-PSV、CRA-EDV、CRA-RI、OA-PSV、OA-EDV、OA-RI及联合检测诊断T2DM患者发生合并视网膜病变的AUC均>0.70,具有一定的诊断价值,且联合检测诊断效能最高.增生型CRA-PSV、CRA-EDV、OA-PSV及OA-EDV低于非增生型(P<0.05),CRA-RI及OA-RI高于非增生型(P<0.05);绘制ROC曲线结果显示,CRA-PSV、CRA-EDV、CRA-RI、OA-PSV、OA-EDV、OA-RI及联合检测评估T2DM患者合并视网膜病变程度的AUC均>0.70,具有一定的评估价值,且联合检测评估效能最高.结论 眼动脉超声血流参数可为T2DM合并视网膜病变患者诊断提供依据,并能有效评估患者视网膜病变严重程度.
Diagnostic Value of Ophthalmic Artery Ultrasonic Blood Flow Parameters in T2DM with Retinopathy and Its Assessment of the Degree of Lesion
Objective To investigate the diagnostic value of ophthalmic artery ultrasonic blood flow parameters in type 2 diabetes mellitus(T2DM)complicated with retinopathy and its assessment of the severity of the lesion.Methods A retrospective study was conducted;80 patients with T2DM complicated with retinopathy admitted to the hospital from March 2023 to March 2024 were selected as the study subjects(the lesion group),and another group of patients with simple T2DM who received treatment during the same period was established as the non-lesion group.The ophthalmic artery ultrasonic blood flow parameters[peak systolic velocity(PSV),end diastolic velocity(EDV),resistance index(RI)of central retinal artery(CRA),and PSV,EDV and RI of ophthalmic artery(OA)]were measured and compared between the 2 groups;the diagnostic value of ophthalmic artery ultrasonic blood flow parameters in the occurrence of retinopathy in T2DM patients was analyzed using receiver operating characteristic(ROC)curves.Based on the severity of retinopathy in T2DM patients,patients with proliferative retinopathy were included in the proliferative group(37/80),and patients with non-proliferative retinopathy were included in the non-proliferative group(43/80);ROC was used to analyze the value of ophthalmic artery ultrasonic blood flow parameters in determining the severity of retinopathy in T2DM patients.Results The CRA-PSV,CRA-EDV,OA-PSV and OA-EDV in the lesion group were lower than those in the non-lesion group(P<0.05),whereas the CRA-RI and OA-RI were higher than those in the non-lesion group(P<0.05);the results of ROC curve showed that CRA-PSV,CRA-EDV,CRA-RI,OA-PSV,OA-EDV,OA-RI,and their combined detection had AUC values greater than 0.70 for diagnosing T2DM patients with retinopathy,indicating their diagnostic value.Among them,the combined detection had the highest diagnostic efficiency.CRA-PSV,CRA-EDV,OA-PSV and OA-EDV in the proliferative group were lower than those in the non-proliferative group(P<0.05),whereas CRA-RI and OA-RI were higher than those in the non-proliferative group(P<0.05);the results of ROC curve demonstrated that CRA-PSV,CRA-EDV,CRA-RI,OA-PSV,OA-EDV,OA-RI,and their combined detection had AUC values greater than 0.70 for assessing the severity of retinopathy in T2DM patients,indicating their evaluative value.Among them,the combined detection exhibited the highest evaluative efficiency.Conclusion The findings suggest that ophthalmic artery ultrasonic blood flow parameters have potential value in the diagnosis of T2DM complicated with retinopathy and can effectively assess the severity of retinopathy in these patients.
