颗粒状联合片状肋软骨在唇裂鼻畸形鼻基底塌陷矫正中的临床应用

摘要
目的 探讨颗粒状肋软骨联合片状肋软骨在唇裂鼻畸形鼻基底塌陷矫正中的临床应用效果.方法 自 2021 年7 月至 2023年 6 月,重庆两江新区人民医院整形外科-医美抗衰中心采用颗粒状肋软骨联合片状肋软骨对 12 例唇裂鼻畸形鼻基底塌陷的患者进行矫正.结果 12 例患者术后获随访 1～18 个月,所有患者鼻翼基底塌陷、鼻小柱基底塌陷、鼻槛塌陷及鼻小柱偏斜、鼻孔大小不对称等情况均得到明显改善.根据术后随访照片及患者自诉,术后 18 个月较术后 6 个月鼻翼基底有所降低,考虑为填充的肋软骨颗粒部分吸收、肌纤维牵拉变薄所致;所有患者均对术后效果满意.结论 采用颗粒状联合片状肋软骨可有效矫正唇裂鼻畸形中鼻基底塌陷效果确切有效,值得临床推广应用.
Abstract
Objective To investigate the clinical effect of granular costal cartilage combined with flake costal cartilage in the cor-rection of nasal base collapse of cleft lip nose deformity.Methods From July 2021 to June 2023,granular costal cartilage combined with flake costal cartilage was used to correct 12 patients with nasal base collapse of cleft lip nose deformity.Results All 12 patients were fol-lowed up for 1 to 18 months.The nasal alar base collapse,nasal columella base collapse,nasal sill collapse,nasal columella deviation and nostril size asymmetry were significantly improved in all patients.The nasal alar base of 12 patients at 18 months after operation was lower than that at 6 months,which was considered to be caused by partial absorption of filled costal cartilage particles,muscle traction and thin-ning.All patients were satisfied with the efficacy.Conclusion The use of granular combined with flake costal cartilage can effectively correct the collapse of nasal base in the nasal deformities of cleft lip,and the clinical application effect is accurate and effective,which is worth popularizing.
