目的 探究调Q 1064 nm Nd:YAG激光联合或单纯果酸治疗黄褐斑的临床疗效及安全性.方法 自 2017 年1 月至 2022 年 12 月于清华大学附属北京清华长庚医院(清华大学临床学院)整形外科就诊的 84 例黄褐斑患者按照治疗方法分为观察组和对照组,对照组(44 例)给予单纯果酸治疗(每 4 周治疗 1 次,共治疗 10 次,为 2 个疗程).观察组(40 例)给予果酸联合调Q激光治疗;激光治疗将根据患者的皮肤状况穿插进行于果酸治疗期间(每 4 周治疗 1次,4 次为 1 个疗程).分别于治疗前、全部治疗结束后 6 个月对患者的黄褐斑面积和严重指数(melasma area severity index,MASI)进行疗效统计,并记录不良反应发生情况.结果 治疗后两组患者MASI评分均降低,观察组MASI评分下降率(41.23±12.34)%高于对照组(26.19±7.84)%,差异有统计学意义(P<0.05);观察组与对照组临床治疗有效率(72.5%和 45.45%)以及患者满意率(65%和 40.90%)比较,差异均具有统计学意义(P<0.05).治疗后所产生的刺痛、瘙痒和结痂等不良反应均在对症处理后消失,无导致治疗停止的严重不良反应出现.结论 采用果酸联合调Q 1064 nm Nd:YAG激光与单纯应用果酸治疗面部黄褐斑均具有确切的治疗效果,但联合治疗效果更佳、安全性较高,并且可获得更高的满意度,值得临床推广应用.
Efficacy analysis of melasma treatment using Q-switched 1064 nm Nd:YAG laser combined with glycolic acid
Objective To explore the clinical efficacy and safety of Q-switched 1064 nm Nd:YAG laser combined with or stan-dalone treatment of glycolic acid for melasma.Methods From January 2017 to December 2022,84 patients with melasma who visited the Department of Plastic Surgery of Beijing Tsinghua Chang Gung Hospital(School of Clinical Medicine,Tsinghua University)were divided into observation and control groups according to treatment methods.The control group(44 patients)was given glycolic acid treatment alone(once every four weeks,a total of 10 treatments,for two courses).The observation group(40 patients)was treated with glycolic acid com-bined with Q-switched laser;laser treatment was interspersed during the treatment period of glycolic acid according to the patient's skin condition(once every four weeks,four treatments for one course).The area and severity of melasma(melasma area severity index,MASI)were statistically analyzed before treatment and six months after all treatments,and the occurrence of adverse reactions was recorded.Results After treatment,the MASI scores of both groups of patients decreased,and the decrease rate of the MASI score in the observation group(41.23±12.34)%was higher than that in the control group(26.19±7.84)%,the difference was statistically significant(P<0.05);the clinical treatment effective rate(72.5%and 45.45%)and patient satisfaction rate(65%and 40.90%)of the observation group compared with the control group were statistically significant(P<0.05).Adverse reactions such as sting,itch,and scab that occurred after treatment disap-peared after symptomatic treatment,and no serious adverse reactions leading to treatment discontinuation occurred.Conclusion The use of glycolic acid combined with Q-switched 1064 nm Nd:YAG laser and the single application of glycolic acid have definite therapeutic ef-fects on facial melasma,but the combined treatment has better effects,higher safety,and can obtain higher satisfaction.It is worth promot-ing in clinical practice.
