小切口联合2%碘酊注射治疗面部皮脂腺囊肿的临床探讨

摘要
目的 探讨小切口联合 2%碘酊注射治疗面部皮脂腺囊肿的临床效果.方法 自 2019 年 7 月至 2021 年 6 月,苏州明基医院美容整形外科门诊收治 79 例面部皮脂腺囊肿患者,按随机数字表法分为观察组(n=41)和对照组(n=38).观察组采用小切口联合 2%碘酊注射治疗;对照组采用沿皮纹切口囊肿整体全切治疗.记录两组患者手术时长.术后随访 6 个月,观察皮脂腺囊肿局部复发情况,采用温哥华瘢痕量表(vancouver scar scale,VSS)对术后瘢痕进行评分,并调查患者满意度.结果 79例患者术后均未见局部复发.手术时长:观察组为(10.14±2.23)min,对照组为(15.78±4.66)min,两组患者手术时长比较,差异有统计学意义(P＜0.05).VSS评分:观察组为(0.44±0.50)分,对照组为(1.71±1.95)分,两组患者评分比较,差异有统计学意义(P＜0.05).观察组满意率为 100%,对照组满意率为 97.37%,两组比较,差异无统计学意义(P>0.05).两组患者满意度构成比的比较,观察组"非常满意"的占比更高,差异有统计学意义(P＜0.05).结论 小切口联合 2%碘酊注射术治疗面部皮脂腺囊肿具有简便、安全、瘢痕较小等优点,同时 2%碘酊注射对囊壁的处理也能避免复发.
Abstract
Objective To explore the clinical application effect of the small incision combined with 2%iodine tincture injection treating of facial sebaceous cysts.Methods Seventy-nine patients with facial sebaceous cyst were randomly divided into an observation group(n=41)and a control group(n=38)using a number table method.The observation group was treated with a small incision combined with 2%iodine tincture injection.The control group was treated with total cystectomy through a straight incision.Observe the duration of surgery for both groups of patients.Follow up for 6 months after surgery to observe the local recurrence of sebaceous cyst,and score the postoperative scars using the vancouver scar scale(VSS).Conduct a patient satisfaction survey.Results No local recurrence occurred in 79 cases.The observation group had a surgical duration of(10.14±2.23)min,while the control group had a surgical duration of(15.78±4.66)min.The difference was statistically significant(P＜0.05).The VSS score of the observation group was 0.44±0.50,while the VSS score of the control group was 1.71±1.95,with a statistically significant difference(P＜0.05).The satisfaction rate of the observation group was 100%,while the satisfaction rate of the control group was 97.37%.There was no statistically significant difference between the two groups.The comparison of the composition ratio of satisfaction between the two groups shows that the observation group had a higher proportion of"very satisfied",and the difference was statistically significant(P＜0.05).Conclusion The combination of the small inci-sion and 2%iodine tincture injection for treating facial sebaceous cysts has the advantages of simplicity,safety,and a small scar.At the same time,the treatment of the cyst wall with 2%iodine tincture injection can also avoid recurrence.
