摘要
目的 评价臭氧皮下注射对椎旁神经阻滞治疗胸背部带状疱疹后神经痛患者疗效的影响.方法 回顾性分析许昌市中心医院疼痛科2021年10月1日至2022年12月31日收治的带状疱疹后神经痛患者77例,病程≤6个月,视觉模拟评分(VAS)≥6分.按治疗方法不同分为2组,神经阻滞组(37例)采用椎旁神经阻滞治疗,联合组(40例)采用臭氧皮下注射联合椎旁神经阻滞治疗.2组治疗均为1次/周,共治疗4次,且均辅以基础药物治疗.比较两组患者治疗前及治疗后7、14、28d的VAS评分;比较两组患者治疗前及治疗后28 d的SF-36评分.观察治疗期间及治疗后1周内并发症发生情况.结果 2组患者治疗前VAS、SF-36评分差异均无统计学意义(P均＞0.05);2组患者治疗后各时点VAS评分均较治疗前降低(P均＜0.05);联合组VAS评分治疗后7、14d均低于神经阻滞组[(4.5±0.6)比(4.9±0.6),(3.1±0.5)比(3.5±0.6),P均＜0.05].联合组治疗后28 d的活力(VT)、社会功能(SF)、情感职能(RE)和心理健康(MH)4个维度较神经阻滞组均改善明显(P均＜0.05).结论 臭氧皮下注射可增强椎旁神经阻滞治疗带状疱疹后神经痛患者的镇痛效果,提高患者活力,社会功能,情感职能,心理健康方面的生活质量.
Abstract
Objective To evaluate the effects of ozone subcutaneous injection on paravertebral nerve block for patients with chest and back postherpetic neuralgia.Methods Seventy-seven patients with chest and back postherpetic neuralgia,duration of disease≤6 months,visual analogue scale(VAS)≥6,in the Department of Pain Medicine,Xuchang Central Hospital from October 1,2021 to December 31,2022 were analyzed retrospectively.All patients were divided into two groups according to the different treatment methods.Thirty-seven patients received paravertebral nerve block in the nerve block group,and 40 patients received paravertebral nerve block combined with ozone subcutaneous injection in the combined group.Both groups were treated once a week for a total of 4 times,and all patients were supplemented with basic drug therapy.VAS before the treatment and 7,14 and 28 days after the treatment,SF-36 score before the treatment and 28 days after the treatment were compared respectively,and the complications were observed during the treatment and 1 week after the treatment.Results VAS,SF-36 score were identical before the treatment between the two groups(all P＞0.05).VAS was decreased at each time point after the treatment than that before the treatment in two groups(all P＜0.05),and were lower in the combined group than that in the nerve block group at 7,14 days after the treatment[(4.5±0.6)vs.(4.9±0.6),(3.1±0.5)vs.(3.5±0.6),all P＜0.05].At 28 days after the treatment,the dimensions of vitality(VT),social functioning(SF),role emotional(RE)and mental health(MH)in SF-36 score were significantly better in the combined group than those in the nerve block group(all P＜0.05).Conclusions Ozone subcutaneous injection can enhance the analgesic effects for paravertebral nerve block in patients with postherpetic neuralgia and improve the quality of life in terms of vitality,social function,emotional function and mental health.
