摘要
目的 评价Clark安适护理结合音乐放松疗法对慢性神经痛患者疼痛程度、心理状态的影响.方法 收集2021年6月1日至2022年6月3日在吉林省人民医院疼痛科住院治疗的慢性神经痛患者88例,年龄＞18岁,入院时疼痛数字等级评分(NRS)≥5分,采用随机数字表法将其随机分为常规护理组和护理干预组,每组44例.常规护理组患者住院期间给予常规护理,护理干预组患者给予Clark安适护理联合音乐放松干预.记录两组患者入院时、住院10d的NRS评分、焦虑分量表(HADS-A)评分、抑郁分量表(HADS-D)评分及匹兹堡睡眠质量指数(PSQI).比较两组患者住院10d内的镇痛药物使用量.结果 两组患者一般资料差异均无统计学意义(P均＞0.05).住院10d时,两组患者NRS评分均较人院时降低[(3.1±0.8)分比(6.5±1.0)分,t=45.47,P＜0.001;(4.6±0.9)分比(6.5±1.0)分,t=32.23,P＜0.001],且护理干预组NRS评分低于常规护理组(t=-8.27,P＜0.001).护理干预组患者住院10 d内的盐酸曲马多使用量低于常规护理组[(103.2±45.3)mg比(136.3±54.2)mg,t=-3.10,P=0.003].住院10 d时,两组患者HADS-A、HADS-D、PSQI评分均较入院时降低(P均＜0.05),且护理干预组各评分均低于常规护理组(P均＜0.05).结论 Clark安适护理结合音乐放松疗法可缓解慢性神经痛患者的疼痛程度、改善心理状态.
Abstract
Objective To evaluate the effects of Clark comfort nursing combined with music relaxation therapy on pain degree and psychological state in patients with chronic neuralgia.Methods Eighty-eight patients with chronic neuralgia in the Department of Painology,Jilin Provincial People's Hospital from June 1,2021 to June 3,2022,aged 18 years and older,numerical rating scale(NRS)≥5,were randomly divided into nursing intervention group and routine nursing group,44 cases in each group.The patients received routine nursing during hospitalization in the routine nursing group,and the patients received Clark comfort nursing combined with music relaxation intervention in the nursing intervention group.NRS,anxiety subscale(HADS-A)score,depression subscale(HADS-D)score and Pittsburgh sleep quality index(PSQI)were recorded on admission and 10 days after hospitalization.The dosage of analgesics was recorded within 10 days of hospitalization in the two groups.Results There was no significant difference in general data between the two groups(all P＞0.05).On the 10th day after hospitalization,NRS was lower than that of admission in the two groups[(3.1±0.8)vs.(6.5±1.0),t=45.47,P＜0.001;(4.6±0.9)vs.(6.5±1.0),t=32.23,P＜0.001],and NRS was lower in the nursing intervention group than that in the routine nursing group(t=-8.27,P＜0.001).On the 10th day after hospitalization,HADS-A,HADS-D and PSQI were significantly decreased in the two groups(all P＜0.05),and the scores were lower in the nursing intervention group than those in the routine nursing group(all P＜0.05).The dosage of tramadol hydrochloride within 10 days was lower in the nursing intervention group than that in the routine nursing group[(103.2±45.3)mg vs.(136.3±54.2)mg,t=-3.10,P=0.003].Conclusion Clark comfort nursing combined with music relaxation therapy can relieve the pain degree and improve the psychological state of patients with chronic neuralgia.
