带状疱疹后神经痛患者外周血中Th17细胞向Treg细胞转化导致免疫失衡的研究

摘要
目的 研究健康人群与带状疱疹后神经痛(PHN)患者外周血中Foxp3+IL-17+中间型细胞与Treg细胞以及Th17细胞间数量变化的相关性.方法 选择2022年8月至2023年11月在青岛市市立医院疼痛科住院治疗的48例PHN患者纳入研究(PHN组),男22例,女26例.以22例同期门诊健康体检者作为对照组.采用流式细胞(FCM)技术检测两组外周血中Treg细胞、Foxp3+IL-17+中间型细胞以及Th17细胞的数量变化,采用t检验分析对照组与PHN组各细胞亚群数量差异,采用Pearson相关性分析对照组和PHN组外周血中Foxp3+IL-17+中间型细胞与Treg细胞和Th17细胞的数量变化进行相关性分析.结果 与对照组相比,PHN组外周血中Foxp3+IL-17+中间型细胞数量明显增加(P＜0.05);Treg细胞的数量也明显增加(P＜0.05);Treg/Th17比值显著增加(P＜0.05);PHN组中,Th17细胞与Foxp3+1L-17+中间型细胞间的数量呈负相关趋势(r=-0.760),但差异无统计学意义(P＞0.05);Treg细胞与Foxp3+IL-17+中间型细胞的数量呈显著的正相关(r=0.900,P＜0.05).结论 PHN患者外周血中可能存在Th17细胞向Treg细胞的转化,这种转化导致Treg/Th17的失衡,造成的免疫功能紊乱可能影响PHN的病程转归与发展.
Abstract
Objective To study the correlation between changes of Foxp3+IL-17+intermediate cells,Treg cells,and Th17 cells in peripheral blood of healthy and PHN populations.Methods A total of 48 patients with PHN in the Department of Painology,Qingdao Municipal Hospital from August 2022 to November 2023 were studied,including 22 males and 26 females(PHN group),and other 22 healthy subjects as control group.The changes of Treg cells,Foxp3+IL-17+intermediate cells and Th17 cells in peripheral blood were detected by flow cytometry(FCM)in the two groups,and the data were analyzed.The correlation between Foxp3+IL-17+intermediate cells,Treg cells and Th17 cells in peripheral blood were analyzed by Pearson correlation analysis.Results Compared with the control group,the number of Foxp3+IL-17+intermediate cells,Treg cells and Treg/Th17 ratio was significantly increased in PHN group(all P＜0.05),the Th17 cells and Foxp3+IL-17+intermediate cells showed a negative correlation trend(r=-0.760,P＞0.05),and the number of Treg cells was positively correlated with Foxp3+IL-17+intermediate cells(r=0.900,P＜0.05).Conclusion The conversion of Th17 cells to Treg cells in peripheral blood of PH N patients leads to the imbalance of Treg/Th17,and these immune dysfunction may affect the course and development of PHN.
