摘要
目的 制定符合中国的临床麻醉与科研评估需求的麻醉后监测治疗室(PACU)内麻醉复苏质量评分量表,并验证其稳定性及可靠性.方法 经讨论制定符合国内临床麻醉与科研评估需求的多维度麻醉后复苏质量评分量表,并经麻醉学专家组对量表的内容效度、稳定性、功能性、实用性等进行讨论后形成预量表.按纳入标准选择2022年10月1日至2023年2月28日河北医科大学第四医院PACU全麻苏醒管理患者103例,采用本预量表进行PACU离室评估,并对其进行项目分析、探索性因子分析、信度检验以验证其可信度和稳定性.结果 修订版量表的条目分析结果显示,量表的各条目与量表总分同质性高;临界比值法和相关系数法分析结果发现,高分组与低分组间差异有统计学意义(P＜0.05),活动与意识条目得分与最终量表总分之间相关系数差异有统计学意义(P＜0.05);量表的Cronbach's α系数为0.647;剔除任一项目后Cronbach's α系数均在0.5以上.结论 本麻醉苏醒质量评分量表可从多维度评估麻醉后患者的复苏质量,鉴别度高,信度良好,量表结构稳定,可在临床推广.
Abstract
Objective To develop a post-anaesthesia care unit(PACU)post-anaesthesia resuscitation quality rating scale that meets the needs of clinical anesthesia and scientific research evaluation in China,and to verify its stability and reliability.Methods After discussion,the domestic multi-dimensional post-anesthesia resuscitation quality rating scale was formulated,and the content validity,stability,functionality and practicability of the scale were discussed by the expert group of anesthesiology.According to the inclusion criteria,103 patients with general anesthesia recovery management in PACU of the Fourth Hospital of Hebei Medical University from October 1,2022 to February 28,2023 were selected,and the PACU was scored by this pre-scale,and its reliability and stability were verified by item analysis,exploratory factor analysis and reliability test.Results The results of item analysis of the pretest scale showed that the items of the pretest scale were highly homogeneous with the total score of the scale.There was significant difference between the high score group and the low score group(P＜0.05).The correlation coefficient between the score of each item and the total score of the final pretest scale was statistically significant(P＜0.05),and each item of the revised scale had good discrimination.The Cronbach's α coefficient of the pretest scale was 0.647.After excluding any item,the Cronbach's α coefficient was above 0.5,indicating good internal consistency,high stability and satisfactory reliability of the scale.Conclusion The anesthesia recovery quality assessment scale developed in this study can evaluate the recovery quality of anesthesia patients from multiple dimensions,with high discrimination,high reliability and stable scale structure,which can be widely used in clinical practice.
