超声引导下脊神经根脉冲射频联合臭氧注射治疗不同病程带状疱疹后神经痛患者的疗效

摘要
目的 探讨超声引导下脊神经根脉冲射频联合臭氧注射治疗不同病程带状疱疹后神经痛的疗效.方法 回顾性分析首都医科大学宣武医院疼痛科2019年1月至2021年10月接受超声引导下脊神经根脉冲射频联合臭氧注射治疗的带状疱疹后神经痛患者118例,根据患者就诊时的病程分为1个月＜病程≤3个月组(A组),3个月＜病程≤6个月组(B组),病程＞6个月组(C组).比较患者治疗前与治疗后1d、1个月、3个月时的疼痛数字等级评分(NRS).比较治疗后1、3个月时患者疼痛缓解效果.采用欧洲五维健康量表(EQ-5D-5L)评估治疗后1、3个月时的生活质量改善情况.比较治疗并发症发生情况.结果 与治疗前比较,治疗后1d、1个月、3个月时3组患者的NRS均降低,差异均有统计学意义(P均＜0.001).与A、B两组比较,治疗后1 d、1个月、3个月时C组患者的NRS均较高,且差异均有统计学意义(P均＜0.05).与A、B两组比较,治疗后1、3个月时C组患者NRS缓解优良率均较低,差异均有统计学意义(P均＜0.05).与A、B两组比较,治疗后1、3个月时,C组患者生活质量改善优良率均较低,差异均有统计学意义(P均＜0.05).患者在治疗期间及治疗后随访时均没有发生穿刺部位感染、神经损伤和原疼痛区域的感觉异常等情况.结论 脊神经根脉冲射频联合臭氧注射均可安全、有效治疗各病程带状疱疹后神经痛患者,并随病程变长,疗效也会变差.
Abstract
Objective To investigate the therapeutic effect of ultrasound-guided spinal nerve root pulse combined with ozone injection on different stage of postherpetic neuralgia.Methods A retrospective analysis of 118 patients with postherpetic neuralgia treated by ultrasound-guided spinal nerve root pulse radiofrequency combined with ozone injection in the Department of Painology,Xuanwu Hospital,Capital Medical University from January 2019 to October 2021,was performed.The patients were divided into 1 month＜stage≤3 months group(group A),3 months＜stage≤6 months group(group B),and state＞6 months group(group C),according to the stage of the disease at the treatment time.Numerical rating scale(NRS)was compared before treatment and 1 day,1 month and 3 months after the treatment.The pain relief effect was compared at 1 and 3 months after treatment.The improvement of quality of life was assessed by EQ-5D-5L at 1 and 3 months after the treatment.The complications were compared.Results Compared with before treatment,NRS were significantly decreased in the three groups at 1 day,1 month and 3 months after treatment(all P＜0.001),and group A,B was better than group C(all P＜0.05).Compared with group A,the excellent and good rates of NRS and quality of life were lower in group C at 1 and 3 months after the treatment(P＜0.05).Conclusion Ultrasonic guided spinal nerve root pulsed radiofrequency combined with ozone injection is an effective treatment method for postherpetic neuralgia,and suitable for various stages of postherpetic neuralgia.
