Effect of Soluble Triggering Receptor Expressed on Myeloid Cells 1 and Lactic Acid on Prognosis of Children with Refractory Mycoplasma Pneumonia
Objective To investigate the effect of soluble triggering receptor expressed on myeloid cells 1(sTREM-1)and lactic acid(LAC)on prognosis of children with refractory mycoplasma pneumonia(RMP).Methods Ninety-nine children with RMP admitted to Huaian Huaian Hospital from January 2021 to February 2024 were selected as the study subjects.According to the prognosis of 1-month follow-up,children were divided into the good prognosis group(n=78)and the poor prognosis group(n=21).The clinical data of the children were collected,and the influencing factors of prognosis of children with RMP was explored by multivariate Logistic regression analysis.ROC curve was drawn to analyze the predictive value of sTREM-1,LAC and their combination for poor prognosis of children with RMP.Results Compared with those in the good prognosis group,the poor prognosis group showed longer disease duration,higher Pediatric Logistic Organ Dysfunction Score 2(PELOD-2)score,percentage of severe cases,CRP,sTREM-1 and LAC,and lower oxygenation index(OI)(P<0.05).The multivariate Logistic regression analysis results showed that,PELOD-2 score[OR=4.314,95%CI(1.673-11.125)],disease severity[OR=6.391,95%CI(1.518-26.904)],sTREM-1[OR=3.199,95%CI(1.190-8.603)],and LAC[OR=2.640,95%CI(1.068-6.527)]were independent influencing factors of the prognosis in children with RMP(P<0.05).ROC curve analysis result showed that,the AUC of sTREM-1,LAC and their combination for predicting poor prognosis of children with RMP was 0.893[95%CI(0.769-0.963)],0.792[95%CI(0.675-0.882)],0.921[95%CI(0.823-0.987)],respectively.Conclusion sTREM-1,LAC are independent influencing factors of the prognosis in children with RMP,and both of them have a certain predictive value for poor prognosis of children with RMP,while their combination has higher predictive value.
