目的 采用健康测量工具选择的共识标准(COSMIN)对高血压健康促进行为的评估工具进行系统评价.方法 计算机检索Embase、Web of Science、PubMed、Cochrane Library、中国生物医学文献服务系统、中国知网、万方数据知识服务平台中关于高血压健康促进行为评估工具的开发、信效度检验的文献,检索时限为建库至2024-05-25.由两名研究生独立进行文献筛选与资料提取.采用COSMIN偏倚风险评价清单评价纳入评估工具的方法学质量,根据COSMIN质量准则评价纳入评估工具的测量学属性质量.应用改良版定量系统评价证据分级(GRADE)方法评价证据等级,根据测量学属性质量评价结果及证据等级确定评估工具的推荐级别.结果 最终纳入文献14篇,共涉及14个高血压健康促进行为评估工具,其中运动行为改变评定量表(MCPAQ)、高血压患者限盐饮食行为改变评定量表(MCRSDH)、运动决策平衡修订量表(DBS)、中文版高血压自我护理量表(HBPSCP)、高血压自我护理量表(HTN-SCP)、健康促进生活方式量表-Ⅱ(HPLP-Ⅱ)中文版为高血压健康促进行为评估工具在不同国家的跨文化验证,高血压病人自我管理行为测评量表(HPSMBRS)、高血压病人自我管理量表、糖尿病-高血压患者行为测量工具、高血压自我护理概况(HBP SCP)、实现健康生活方式的倾向性量表(PAHLS)、青年高血压患者疾病知识和行为量表、中青年高血压病人自我管理行为量表、高血压患者主动健康行为量表(AHBS)为高血压健康促进行为评估工具的开发.MCPAQ、MCRSDH、HTN-SCP、HPLP-Ⅱ中文版、高血压病人自我管理量表、AHBS的推荐级别为C级,DBS、中文版HBPSCP、HPSMBRS、糖尿病-高血压患者行为测量工具、HBP SCP、PAHLS、青年高血压患者疾病知识和行为量表、中青年高血压病人自我管理行为量表的推荐级别为B级.结论 HPSMBRS的测量学属性(结构效度、内部一致性)较好,可暂时优先推荐应用于高血压健康促进行为评估.
Systematic Review of Hypertension Health Promotion Behavior Assessment Tools Based on COSMIN
Objective To systematically evaluate the hypertension health promotion behavior assessment tools by using Consensus Criteria for the Selection of Health Measurement Instruments(COSMIN).Methods Literature on the development of evaluation tools for hypertension health promotion behavior and their reliability and validity were searched by computer in Embase,Web of Science,PubMed,Cochrane Library,SinoMed,CNKI,and Wanfang Data.The retrieval deadline was from database establishment to May 25,2024.Literature screening and data extraction were conducted independently by two graduate students.The methodological quality of the included assessment tools was evaluated using the COSMIN bias risk assessment checklist,and the quality of the measurement attributes of the included assessment tools was evaluated according to the COSMIN quality criteria.The Grading of Recommendation,Assessment,Development and Evaluation(GRADE)method was used to evaluate the evidence grade,and the recommended level of the evaluation tool was determined according to the quality evaluation results of measurement attributes and the evidence grade.Results Finally,14 articles were included,involving a total of 14 hypertension health promotion behavior assessment tools.Among them,Measuring Change in Physical Activity Questionnaire(MCPAQ),Measuring Change in Restriction of Salt(Sodium)in Diet in Hypertensives(MCRSDH),Decisional Balance Scale(DBS),Chinese version of Hypertension Selfcare Profile(HBPSCP),Hypertension Self Care Profile Questionnaire(HTN-SCP),Health Promoting Lifestyle Profile-Ⅱ(HPLP-Ⅱ)Chinese version were cross-cultural validation of hypertension health promotion behavior assessment tools in different countries,Hypertension Patients Self-Management Behavior Rang Scale(HPSMBRS),Self Management Scale for Patients with Hypertension,Behaviour Measurement Instrument of Patients with Diabetes Mellitus and Hypertension,Hypertension Self-care Profile(HBP SCP),Propensity to Achieve Healthy Lifestyle Scale(PAHLS),Youth Hypertension Patient Disease Knowledge and Behavior Scale,Self-Management Behavior Scale in Young and Middle-Aged Patients with Hypertension and Active Health Behavior Scale(AHBS)were about the development of hypertension health promotion behavior assessment tools.The recommended level of MCPAQ,MCRSDH,HTN-SCP,HPLP-Ⅱ Chinese version,Self Management Scale for Patients with Hypertension and AHBS was grade C.The recommended level of DBS,Chinese version of HBPSCP,HPSMBRS,Behaviour Measurement Instrument of Patients with Diabetes Mellitus and Hypertension,HBP SCP,PAHLS,Youth Hypertension Patient Disease Knowledge and Behavior Scale,Self-Management Behavior Scale in Young and Middle-Aged Patients with Hypertension was grade B.Conclusion HPSMBRS has good measurement properties(structural validity,internal consistency),and can be recommended for the evaluation of health promotion behavior in hypertension.
