目的 构建基于自我调节常识模型(CSM)的冠心病患者心脏运动康复干预方案.方法 成立研究小组,计算机检索UpToDate、英国国家卫生与临床优化研究所网站、美国国立指南库网站、国际指南协作网、医脉通网站、JBI循证卫生保健中心数据库、PubMed、Embase、Web of Science、Cochrane Library、BMJ最佳临床实践、加拿大安大略省注册护士协会网站、中国知网、万方数据知识服务平台、中国生物医学文献数据库中关于基于CSM的冠心病患者心脏运动康复的文献.检索时限从建库至2023年10月.通过文献分析制定基于CSM的冠心病患者心脏运动康复干预方案初稿.2023年11月—2024年2月通过邮箱和微信统一发放、回收问卷,采用德尔菲法进行两轮专家函询,构建基于CSM的冠心病患者心脏运动康复干预方案.采用专家函询问卷回收率评估专家积极程度,采用专家权威系数评估专家权威程度,采用变异系数和肯德尔和谐系数评估专家意见协调程度,采用重要性评分评估专家意见集中程度.结果 第1轮和第2轮专家函询均发放问卷16份,回收有效问卷16份,问卷有效回收率均为100.0%.第1轮专家函询的熟悉程度系数为0.938,判断依据系数为0.953,专家权威系数为0.922;第2轮专家函询的熟悉程度系数为0.948,判断依据系数为0.962,专家权威系数为0.934.第1轮专家函询的变异系数为0.070~0.263,肯德尔和谐系数为0.202(P<0.01);第2轮专家函询的变异系数为0.070~0.218,肯德尔和谐系数为0.277(P<0.01).最终形成基于CSM的冠心病患者心脏运动康复干预方案,包括5个干预阶段、7个干预主题、14条干预内容、27条干预措施.结论 本研究构建了基于CSM的冠心病患者心脏运动康复干预方案,其包括5个干预阶段、7个干预主题、14条干预内容、27条干预措施.
Construction of Intervention Program for Cardiac Exercise Rehabilitation in Patients with Coronary Heart Disease Based on Self-Regulation Commonsense Model
Objective To construct an intervention program for cardiac exercise rehabilitation in patients with coronary heart disease based on self-regulation commonsense model(CSM).Methods A research team was established,the literature on cardiac exercise rehabilitation in patients with coronary heart disease based on CSM was retrieved from UpToDate,National Institute for Health and Clinical Excellence website,National Guideline Clearinghouse website,Guidelines International Network,Medlive website,JBI Evidence Based Health Care Centre database,PubMed,Embase,Web of Science,Cochrane Library,BMJ Best Practice,Registered Nurses'Association of Ontario website,CNKI,Wanfang Data,and China Biology Medicine disc.The search period was from inception to October 2023.An initial draft of intervention program for cardiac exercise rehabilitation in patients with coronary heart disease based on CSM was developed through literature review.From November 2023 to February 2024,questionnaires were uniformly distributed and collected via email and WeChat,and two rounds of expert consultation were conducted using the Delphi method to construct the intervention program for cardiac exercise rehabilitation in patients with coronary heart disease based on CSM.The response rate of expert inquiry questionnaires was used to evaluate the level of expert engagement,the expert authority coefficient was used to evaluate the level of expert authority,the coefficient of variation and Kendall's coefficient of concordance were used to evaluate the degree of expert opinion coordination,the importance score was used to evaluate the concentration of expert opinions.Results Sixteen questionnaires were distributed in the first and second rounds of expert consultation,and 16 valid questionnaires were collected,with a valid response rate of 100.0%for both rounds.In the first round of expert consultation,the familiarity coefficient was 0.938,the judgment criteria coefficient was 0.953,and the expert authority coefficient was 0.922.In the second round of expert consultation,the familiarity coefficient was 0.948,the judgment criteria coefficient was 0.962,and the expert authority coefficient was 0.934.In the first round of expert consultation,the coefficient of variation was 0.070-0.263,the Kendall's coefficient of concordance was 0.202(P<0.01);in the second round of expert consultation,the coefficient of variation was 0.070-0.218,the Kendall's coefficient of concordance was 0.277(P<0.01).The final intervention program for cardiac exercise rehabilitation in patients with coronary heart disease based on CSM was established,including 5 intervention stages,7 intervention themes,14 intervention contents,and 27 specific intervention measures.Conclusion This study developed a intervention program for cardiac exercise rehabilitation in patients with coronary heart disease based on CSM,it includs 5 intervention stages,7 intervention themes,14 intervention contents,and 27 specific intervention measures.
