Mediating Effects of Kinesiophobia on Health Literacy and Cardiac Rehabilitation Barriers in Patients with Coronary Heart Disease after PCI
Objective To explore the mediating effects of kinesiophobia on health literacy and cardiac rehabilitation barriers in patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods The patients with coronary heart disease after PCI admitted to the First Affiliated Hospital of Guangxi University of Chinese Medicine,Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine,and Jiangbin Hospital of Guangxi Zhuang Autonomous Region from January to March 2024 were selected as the survey subjects.The patients were surveyed by the General Information Questionnaire,PCI Patients'Health Literacy Scale,Tampa Scale for Kinesiophobia Heart(TSK-SV Heart)and Cardiac Rehabilitation Barriers Scale(CRBS).Pearson correlation analysis was used to explore the correlation between the total scores of PCI Patients'Health Literacy Scale,TSK-SV Heart and CRBS in patients with coronary heart disease after PCI.Mediating variables analysis in PROCESS macro was used to analyze the mediating effects of kinesiophobia on health literacy and cardiac rehabilitation barriers in patients with coronary heart disease after PCI.Results A total of 204 questionnaires were distributed and 202 valid questionnaires were returned,with a valid return rate of 99.0%.The total score of PCI Patients'Health Literacy Scale in patients with coronary heart disease after PCI ranged from 41 to 116 points,with an average of(76.3±15.6)points;the total score of TSK-SV Heart ranged from 34 to 60 points,with an average of(44.4±4.2)points;the total score of CRBS ranged from 32 to 97 points,with an average of(62.7±12.6)points.The total score of CRBS in patients with middle school and below was higher than that in patients with high school/vocational school,college and above(P<0.05).Pearson correlation analysis showed that the total score of PCI Patients'Health Literacy Scale in patients with coronary heart disease after PCI was negatively correlated with the total scores of TSK-SV Heart and CRBS(P<0.05),the total score of TSK-SV Heart was positively correlated with the total score of CRBS(P<0.05).The results of mediating variables analysis in PROCESS macro showed that the direct effect value of health literacy on cardiac rehabilitation disorders in patients with coronary heart disease after PCI was-0.257[95%CI(-0.364--0.151)],accounting for 88.62%of the total effect value.The indirect effect value of kinesiophobia between health literacy and cardiac rehabilitation disorders was-0.033[95%CI(-0.073--0.004)],accounting for 11.38%of the total effect value.Conclusion The level of cardiac rehabilitation disorders in patients with coronary heart disease after PCI is at moderate to high level,health literacy has a detrimental effect on kinesiophobia,and kinesiophobia plays a partial mediating effects between health literacy and cardiac rehabilitation barriers.
