基于磁共振T2加权成像信号强度比值鉴别鼻咽炎与鼻咽癌的临床价值分析

摘要
目的 探讨基于磁共振T2加权成像(T2WI)信号强度比值(signal intensity ratio,SIR)在鉴别鼻咽炎症与鼻咽癌的临床价值.方法 收集2012年7月至2017年12月经病理证实的鼻咽病变患者60例,其中鼻咽炎患者10例,鼻咽癌患者50例,在磁共振轴位T2WI测量鼻咽病灶最大信号强度值(Max-SIR)、最小信号强度值(Min-SIR)、平均信号强度值(Mean-SIR),并计算与椎前肌Mean-SIR的比值,分析鼻咽炎与鼻咽癌的T2WI信号强度比值差异.结果 鼻咽炎与鼻咽癌的Max-SIR(6.4±1.8与4.5±1.0)、Min-SIR(3.9±0.8与2.9±0.8)、Mean-SIR(5.1±1.1与3.7±0.8)差异有统计学意义(P<0.05);Max-SIR、Min-SIR、Mean-SIR鉴别鼻咽炎与鼻咽癌的受试者工作特征(ROC)曲线下面积分别为0.842、0.828、0.868;Max-SIR的最佳诊断阈值为5.38,灵敏度为80%,特异度为86%;Min-SIR的最佳诊断阈值为3.39,灵敏度为80%,特异度为80%;Mean-SIR的最佳诊断阈值为4.58,灵敏度为80%,特异度为88%.结论 磁共振T2WI-SIR简单易行,可以作为鉴别鼻咽炎与鼻咽癌的有效方法.
Abstract
Objective To investigate the clinical value of signal intensity ratio(SIR)based on magnetic resonance T2WI in differentiating nasopharyngitis and nasopharyngeal carcinoma(NPC).Methods Sixty cases with nasopharyngeal lesions confirmed by pathology from July 2012 to December 2017 were collected,including 10 cases with nasopharyngitis and 50 cases with NPC.The maximum signal intensity(Max-SIR),minimum signal intensity(Min-SIR),and mean signal intensity(Mean-SIR)of nasopharyngeal lesions were measured on axial T2WI of MRI,and the ratio with the mean signal value of prevertebral muscle was calculated.The difference of SIR of T2WI between nasopharyngitis and NPC was analyzed.Results The difference of Max-SIR(6.4±1.8 vs 4.5±1.0),the Min-SIR(3.9± 0.8 vs 2.9±0.8),and the Mean-SIR(5.1±1.1 vs 3.7±0.8)between nasopharyngitis and NPC was statistically significant(P<0.05).The area under the ROC curve of Max-SIR,Min-SIR,and Mean-SIR for distinguishing nasopharyngitis from NPC was 0.842,0.828,and 0.868,respectively.The optimal diagnostic threshold of Max-SIR was 5.38,and the sensitivity and specificity were 80%and 86%,respectively.The optimal diagnostic threshold of Min-SIR was 3.39,and the sensitivity and specificity were 80%and 80%,respectively.The optimal diagnostic threshold of Mean-SIR was 4.58,and the sensitivity and specificity were 80%and 88%,respectively.Conclusion MR T2WI-SIR is a simple and easy method to distinguish nasopharyngitis from nasopharyngeal carcinoma.
