摘要
目的 探讨心脏多普勒超声参数对高血压性心脏病(HHD)患者预后的预测价值.方法 选取我院心内科2019年8月至2020年8月收治的106例HHD患者作为研究对象,根据预后情况将106例HHD患者分为存活组、死亡组,比较2组一般资料、心脏多普勒超声参数、生化指标,筛选影响HHD患者预后的危险因素,绘制受试者工作特征(ROC)曲线判断发生死亡时,心脏多普勒超声参数最佳截断值,分析心脏多普勒超声参数对HHD患者预后的预测价值.结果 死亡组年龄、心功能Ⅲ级占比、收缩压水平、左心房内径(LAD)水平高于存活组,左心室射血分数(LVEF)水平低于存活组,差异有统计学意义(P＜0.05);Logistic多因素回归分析显示,年龄、心功能分级、收缩压、LAD、LVEF水平均为HHD患者死亡危险因素(P＜0.05);ROC曲线显示,LAD、LVEF及二者联合预测HHD患者预后情况的曲线下面积分别为0.842、0.883、0.902,灵敏度为83.2％、93.8％、96.8％,特异度为57.6％、51.9％、61.2％.结论 心脏多普勒超声参数对HHD患者的预后情况有一定预测价值,可作为评估HHD患者预后情况的重要依据.
Abstract
Objective To explore the predictive value of cardiac Doppler ultrasound parameters on the prognosis of patients with hypertensive heart disease(HHD).Methods One hundred and six patients with HHD admitted to the Department of Cardiology in our hospital from August 2019 to August 2020 were selected as the research subjects.According to the prognosis,106 patients with HHD were divided into a survival group and a death group.The general information,cardiac Doppler ultrasound parameters,and biochemical indicators of the two groups were compared to screen for risk factors that affect the prognosis of HHD patients.The receiver operating characteristic curve(ROC)was drawn to determine the optimal cutoff value of cardiac Doppler ultrasound parameters when the death occurred.The predictive value of cardiac Doppler ultrasound parameters on the prognosis of HHD patients was analyzed.Results The age,proportion of grade Ⅲ cardiac function,systolic blood pressure level,and left atrial diameter(LAD)level in the death group were higher than those in the survival group,while left ventricular ejection fraction(LVEF)level was lower than those in the survival group,with statistically significant differences(P＜0.05).Logistic multivariate regression analysis showed that age,cardiac function grading,systolic blood pressure,LAD,and LVEF levels were all risk factors for death in HHD patients(P＜0.05).ROC curve showed that the area under the curve of LAD,LVEF and their combination in predicting the prognosis of HHD patients was 0.842,0.883,0.902,sensitivity was 83.2％,93.8％,96.8％,and specificity was 57.6％,51.9％,61.2％,respectively.Conclusion Cardiac Doppler ultrasound parameters have certain predictive value for the prognosis of patients with HHD,and can serve as an important basis for evaluating the prognosis of patients with HHD.
