磁共振成像与多层螺旋CT诊断对胃癌术前N分期及淋巴结转移的诊断效能及影像学特点分析

摘要
目的 分析磁共振成像(MRI)与多层螺旋CT(MSCT)诊断对胃癌术前N分期及淋巴结转移的诊断效能及影像学特点.方法 本研究选取我院2020年1月至2022月12月接收的132例胃癌患者,所选患者均接受MRI扫描与MSCT扫描检查,统计分析MRI扫描与MSCT扫描对胃癌的术前N分期、淋巴结转移情况的诊断准确率.结果 MRI及MSCT对患者术前N分期的诊断结果比较差异无统计学意义(P＞0.05).MRI诊断中,发生淋巴结转移患者的相对表观扩散系数(rADC)低于未发生淋巴结转移的患者组(P＜0.05),MSCT诊断中,发生淋巴结转移患者的rADC与未发生淋巴结转移的患者组比较更高(P＜0.05).ROC曲线分析结果表明,MRI对淋巴结转移诊断的rADC阈值为0.78时曲线下面积为0.809,灵敏度为78.5％,特异度为54.8％.MSCT淋巴结转移诊断的强化程度阈值为36.22 Hu时曲线下面积为0.882,灵敏度为77.3％,特异度为91.8％.结论 MRI与MSCT诊断对胃癌术前N分期及淋巴结转移均具有较高的诊断效能.
Abstract
Objective To analyze the diagnostic efficacy and imaging characteristics of MRI and multislice spiral CT(MSCT)for preoperative N-staging and lymph node metastasis of gastric cancer.Methods One hundred and thirty-two patients with gastric cancer received in our hospital from January 2020 to December 2023 underwent MRI scanning with MSCT scanning examination,and the statistical analysis of the diagnostic accuracy between MRI scanning and MSCT scanning for the preoperative N staging and lymph node metastasis of gastric cancer was performed.Results There was no significant difference between the diagnostic results of MRI and MSCT for preoperative N staging of patients(P＞0.05).MRI diagnosis showed that the relative apparent diffusion coefficient(rADC)of patients with lymph node metastasis was significantly lower than patients without lymph node matastasis(P＜0.05),and MSCT diagnosis showed that the rADC was significantly higher in the group of patients who developed lymph node metastasis than patients who did not(P＜0.05).ROC curve analysis indicated that MRI had an area under the curve of 0.809,sensitivity of 78.5％,and specificity of 54.8％at an rADC threshold of 0.78 for the diagnosis of lymph node metastasis,while the area under the curve was 0.882 at an intensified degree threshold of 36.22 Hu for the diagnosis of MSCT lymph node metastasis,the sensitivity was 77.3％and the specificity 91.8％.Conclusion MRI with MSCT diagnosis has high diagnostic efficacy for both preoperative N stage and lymph node metastasis of gastric cancer.
