经阴道超声在深部浸润型子宫内膜异位症中的诊断价值

摘要
目的 探讨经阴道超声在深部浸润型子宫内膜异位症中的诊断价值.方法 择选2022年2月至2023年2月在我院接受治疗的89例疑似浸润型子宫内膜异位症患者作为研究对象,均实施经腹超声、经阴道超声诊断.以腹腔镜手术病理诊断结果为金标准,计算诊断准确性、灵敏度、特异度.结果 经阴道超声诊断准确率明显高于经腹部超声,差异有统计学意义(P＜0.05).经腹部超声诊断灵敏度63.8％(51/80),特异度55.6％(5/9);经阴道超声诊断灵敏度93.8％(75/80),特异度88.9％(8/9).经阴道超声诊断灵敏度、特异度明显高于经腹部超声,差异有统计学意义(P＜0.05).Kappa检验一致性,Kappa=0.734(P＞0.05).结论 深部浸润型子宫内膜异位症患者采用经阴道超声,能准确定位病变部位,为临床治疗提供依据.
Abstract
Objective To investigate the diagnostic value of transvaginal ultrasonography in deep infiltrating endometriosis.Methods A total of 89 patients with suspected invasive endometriosis who received treatment in our hospital from February 2022 to February 2023 were selected as the study objects,and all of them were diagnosed by transabdominal ultrasound and transvaginal ultrasound.The diagnostic accuracy,sensitivity and specificity were calculated with the pathological diagnosis results of laparoscopic surgery as the gold standard.Results The diagnostic accuracy of transvaginal ultrasound was significantly higher than that of transabdominal ultrasound,and the difference was statistically significant(P＜0.05).The diagnostic sensitivity of transabdominal ultrasound was 63.8％(51/80),and the specificity was 55.6％(5/9).The diagnostic sensitivity of transvaginal ultrasound was 93.8％(75/80),and the specificity was 88.9％(8/9).The diagnostic sensitivity and specificity of transvaginal ultrasound were significantly higher than that of transabdominal ultrasound,and the difference was statistically significant(P＜0.05).The result of the Consistency of Kappa test was Kappa=0.734(P＞0.05).Conclusion Transvaginal ultrasound can accurately locate the lesion in patients with deep infiltrating endometriosis,which is the basis for improving clinical treatment.
