摘要
目的 探究精准经皮肝穿刺技术用于肝Ⅷ段小肝癌和脓肿治疗中的安全性和可行性.方法 选择 2021年 3 月至 2023 年 3 月我院收治的 80 例肝Ⅷ段小肝癌和脓肿患者作为本研究的观察对象,将其按随机数表法分为对照组(40 例)与观察组(40 例),对照组采用腹腔镜联合镜下超声引导的肝穿刺置管引流/射频消融治疗,观察组采用CT联合超声引导的经皮肝穿刺置管引流/射频消融治疗,对 2 组的围手术期指标(穿刺时间、穿刺次数、穿刺成功率、术后下床时间、术后胃肠功能恢复时间)、炎症因子水平、并发症发生情况进行比较.结果 2 组的穿刺次数、穿刺成功率差异无统计学意义(P>0.05),观察组的穿刺时间、术后下床时间和胃肠功能恢复时间均较短(P<0.05);治疗后,观察组的超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、白细胞介素-8(IL-8)水平均低于对照组(P<0.05);2 组的并发症发生率差异无统计学意义(P>0.0.5).结论 CT联合超声引导的精准经皮肝穿刺技术用于肝Ⅷ段小肝癌和脓肿治疗中能有效缩短穿刺时间,有利于患者术后快速康复,并改善机体的炎症因子水平,控制术后并发症的发生,具有较高的安全性与可行性.
Abstract
Objective To investigate the safety and feasibility of the precision percutaneous hepatic puncture technique in the management of small hepatocellular carcinomas and abscesses in segment Ⅷof the liver.Methods Eighty patients with small hepatocellular carcinoma and abscess of liver Ⅷ segment admitted to our hospital between March 2021 and March 2023 were selected as the subjects of this study,and they were divided into the control group(40 cases)and the observation group(40 cases)according to the random method of numerical tables,the control group was treated with laparoscopy combined with microscopic ultrasound-guided hepatic perforation and drainage/radiofrequency ablation,and the observation group was treated with percutaneous hepatic perforation and drainage/radiofrequency ablation with CT combined with ultrasound guidance/radiofrequency ablation treatment,and the perioperative indexes(puncture time,number of punctures,puncture success rate,postoperative time out of bed,postoperative time for recovery of gastrointestinal function),levels of inflammatory factors,and the occurrence of complications were compared between the two groups.Results There was no significant difference in the number of puncture and puncture success rate between the two groups(P>0.05),and the puncture time,postoperative time out of bed,and gastrointestinal function recovery time were shorter in the observation group(P<0.05);after treatment,the levels of ultrasensitive C-reactive protein(hs-CRP),procalcitoninogen(PCT),and interleukin-8(IL-8)in the observation group were lower than those in the control group(P<0.05);the two groups'complication rates were not significantly different(P>0.05).Conclusion CT combined with ultrasound-guided precision percutaneous hepatic puncture for the treatment of small hepatocellular carcinoma and abscess in liverⅧcan effectively shorten the time of puncture,which is conducive to the rapid recovery of patients,and improve the level of inflammatory factors of the body to control the occurrence of postoperative complications,which has high safety and feasibility.
