摘要
目的 探讨卵巢-附件影像报告和数据系统(O-RADS)和改良妇科影像学报告和数据系统(mGI-RADS)诊断附件恶性肿瘤的价值、筛选独立危险因素及其诊断价值分析.方法 回顾性分析 226个肿瘤,其中良性 192 个,恶性 34 个,以病理结果为金标准绘制受试者工作特征(ROC)曲线.对临床及超声参数行回归分析,筛选预测附件恶性肿瘤的独立危险因素.结果 患者年龄、肿瘤最大径、不规则囊壁、不规则或厚薄不均分隔、实性成分≥30%、乳头状凸起≥4个、彩色血流评分3~4分、腹腔积液作为预测附件恶性肿瘤的独立危险因素.O-RADS和mGI-RADS及独立危险因素≥2 个的曲线下面积(AUC)分别为 0.880、0.834、0.868,差异均无统计学意义.结论 O-RADS和mGI-RADS及独立危险因素≥2 个诊断附件恶性肿瘤的价值均较高.
Abstract
Objective To explore the value of ovarian-adnexal reporting and data system(O-RADS)and modified gynecology imaging reporting and data system(mGI-RADS)in diagnosing adnexal malignancies,and to screen independent risk factors and analyze their diagnostic value.Methods A retrospective analysis of 226 adnexal tumors was carried out with 192 cases being benign and 34 cases being malignant.The pathological results were used as the gold standard to draw the receiver operator characteristic curve(ROC).Regression analyses were performed on the clinical and ultrasound parameters of the patients to screen out independent risk factors for predicting adnexal malignant tumors.Results The patients'age,maximum tumor diameter,irregular cystic wall,irregular or uneven thickness of the separation,solid component≥30%,papillae≥4,color Doppler flow score 3-4 points,and ascites were independent risk factors for predicting adnexal malignancy.The area under the curve(AUC)of O-RADS,mGI-RADS,and independent risk factor≥2 were 0.880,0.834,and 0.868,respectively,and the differences were not statistically significant.Conclusion The values of O-RADS,mGI-RADS,and independent risk factor≥2 in diagnosing adnexal malignancies were high.
