3.0T磁共振动态增强联合扩散加权成像对乳腺疾病的临床应用价值

目的 探讨 3.0T磁共振动态增强(DCE-MRI)及扩散加权成像(DWI)对乳腺良恶性疾病的诊断价值.方法 回顾性分析经手术或穿刺病理证实的 55例乳腺疾病患者的病理结果及影像学资料,病例均术前行双乳磁共振成像(MRI)动态增强及DWI检查.分析病灶时间一信号强度曲线(TIC)及表观扩散系数(ADC)值,与病理结果对照,分析DCE-MRI、DWI、DCE-MRI联合DWI对乳腺病变的诊断价值.结果 55 例患者中恶性病例31例,良性病例 24例.DCE-MRI序列诊断符合率为 76%,DWI序列诊断符合率 78%.DCE-MRI联合DWI的诊断敏感度达 94%,特异度为 88%,符合率为 90%.良恶性病变TIC曲线分布示:I型、Ⅲ型曲线统计结果分别为P= 0.002、P =0.038,差异具有统计学意义.结论 DCE-MRI与DWI序列联合分析及TIC曲线分布可提高乳腺良恶性病变诊断的准确度,为临床诊疗提供更可靠的影像学依据.
Abstract
Objective The purpose of evaluating the 3.0T dynamic value is to enhance magnetic resonance imaging(DCE-MRI)and propagation-weighted imaging(DWI)for the diagnosis of benign and malignant breast diseases.Methods A retrospective analysis was conducted on the pathological results and imaging data of 55 patients with breast diseases confirmed by surgery or puncture pathology.All patients underwent dynamic enhanced MRI and DWI examination of both breasts before surgery.The time-signal intensity curve(TIC)and apparent diffusion coefficient(ADC)values of the lesion were analyzed,and compare them with pathological results to analyze the diagnostic value of DCE-MRI,DWI,and DCE-MRI combined with DWI for breast lesions.Results Among the fifty five patients,there were 31 malignant cases and 24 benign cases.The diagnostic accuracy of the DCE-MRI sequence was 76%,and the diagnostic accuracy of the DWI sequence was 78%.The diagnostic sensitivity of DCE-MRI combined with DWI was 94%,the specificity was 88%,and the coincidence rate was 90%.The distribution of TIC curves for benign and malignant lesions shows that the statistical results of type Ⅰ and type Ⅲ curves are P=0.002 and P=0.038,respectively,indicating statistical significance.Conclusion The conjoint analysis of DCE-MRI and DWI sequence and the distribution of the TIC curve can improve the accuracy of diagnosis of benign and malignant breast lesions and provide a more reliable imaging basis for clinical diagnosis and treatment.
