Objective To differentiate histopathology from benign and malignant endometrial lesions by MRI,and the quantitative analysis of apparent diffusion coefficient(ADC)values,histological tumor grades,and clinical stages.Methods A total of 120 women with or without vaginal bleeding and endometrial thickness up to the patho-logical value were prospectively analyzed.Results Forty-eight cases with suspected malignant endometrial lesions and 72 cases with suspected benign endometrial lesions were diagnosed by MRI,and 43 cases with malignant lesions and 77 cases with benign lesions were finally histopathology.The ADC value of malignant lesions was lower than that of benign lesions[(0.79±0.11)×10-3mm2/s vs(1.30±0.19)×10-3mm2/s],and the difference was statistically significant(P<0.05).The sensitivity,specificity,positive predictive value,and negative predictive value of MRI were 100%,93.5%,89.6%,and 100%,respectively.The cut-off value of ADC between benign and malignant lesions was 1.01×10-3mm2/s,with a sensitivity of 100%and a specificity of 91.7%.There was no significant difference in average ADC value among different grades of endometrial cancer histological grading(P>0.05).The most common clinical FIGO stage was stage IA and IB,and there was no significant difference in the average ADC values among the stages(P>0.05).Conclusion DWI and ADC value are effective methods to differentiate benign from malignant endometrial lesions,which have high sensitivity and good specificity,however,it is of little significance in differentiating endometrial cancer histological grading and clinical FIGO staging.
