Objective To investigate the efficacy of ultrasonic transient elastic imaging parameters in evaluating liver fibrosis in hepatitis B patients with nonalcoholic fatty liver disease(NAFLD).Methods The general data of 110 patients with hepatitis B combined with NAFLD admitted to our hospital from May 2020 to June 2022 were retrospectively analyzed.All patients underwent liver biopsy.According to the Guidelines for the Prevention and Treatment of Nonalcoholic Fatty Liver Disease(2018 Updated Edition)and the fibrosis score of Metavir,the patients were divided into F0-1 group(n=42),F2-3 group(n=45)and F4 group(n=23).The liver biochemical indexes(ALT,AST,ALP,GGT,TC,TG)were detected,and the LSM value was detected by a liver transient elastic scanner.Spearman was used to analyze the correlation between LSM and the degree of liver fibrosis,and the receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of LSM for different degrees of liver fibrosis in hepatitis B patients with NAFLD.Results There were significant differences in gender,BMI,ALT,AST,ALP and LSM among the three groups(P<0.05).Spearman correlation analysis showed that LSM was positively correlated with gender,BMI,ALT,AST(r=0.105,r=0.162,r=0.184,r=0.167 and P<0.05).There was no significant correlation with ALP(r=0.019,P>0.05).ROC curve analysis showed that the area under the curve(AUC)of F0-1,F2-3,and F4 in patients with hepatitis B combined with NAFLD diagnosed by LSM were 0.910,0.828,and 0.929.The sensitivity and specificity were 75.0%、91.3%、77.8%、78.3%、82.6%、84.4%.The truncation values were 6.28 kPa,6.98 kPa and 10.45 kPa.Conclusion Ultrasonic transient elastic imaging parameters have good diagnostic efficacy and high stability in evaluating the degree of liver fibrosis in hepatitis B patients with NAFLD.
