不同肝脏背景下超声造影与常规超声对肝血管瘤的诊断价值

摘要
目的 研究超声造影与常规超声对不同肝脏背景下肝血管瘤的诊断价值.方法 经病理或双重影像学证据确诊血管瘤123枚,应用常规超声观察病灶大小、数量、形态、边缘、内部回声、位置、血供等情况;超声造影实时观察病灶动脉期、门脉期、延迟期灌注情况.结果 常规超声正常肝背景诊断符合率86.5％,脂肪肝背景诊断符合率82.6％,肝硬化背景诊断符合率58.3％,不同肝脏背景下诊断符合率差异有统计学意义,且肝硬化背景下诊断符合率显著低于前两者;超声造影正常肝背景下诊断符合率94.2％,脂肪肝背景诊断符合率93.5％,肝硬化背景诊断符合率77.8％,在不同背景下诊断符合率差异无统计学意义.超声造影诊断效能优于常规超声.结论 较于常规超声,超声造影有更好的诊断价值,尤其在声像图不典型时,值得临床推广.
Abstract
Objective To study the diagnostic value of contrast-enhanced ultrasound and conventional ultra-sound in hepatic hemangioma in different hepatic backgrounds.Methods A total of 123 hemangiomas were con-firmed by pathology or dual imaging evidence.Conventional ultrasound was used to observe the size,number,shape,margin,internal echo,location and blood supply of the lesions.The contrast-enhanced ultrasound was used to ob-serve the perfusion in arterial phase,portal phase and delayed phase.Results The coincidence rate of convention-al ultrasound diagnosis in normal liver background was 86.5％,that in fatty liver background was 82.6％,and that in cirrhosis background was 58.3％.There were significant differences in the diagnosis coincidence rate in different liv-er backgrounds,and the diagnosis coincidence rate in cirrhosis background was significantly lower than the previous two.The diagnostic coincidence rate of contrast-enhanced ultrasound was 94.2％in the background of normal liver,93.5％in the background of fatty liver,and 77.8％in the background of cirrhosis.There was no significant differ-ence in the diagnostic coincidence rate in different backgrounds.The diagnostic efficiency of contrast-enhanced ul-trasound is better than that of conventional ultrasound.Conclusion Compared with conventional ultrasound,con-trast-enhanced ultrasound has better diagnostic value,especially in the case of atypical ultrasound images,worthy of clinical promotion.
