摘要
目的 探讨乳腺数字断层合成与动态磁共振成像(MRI)在诊断乳腺良恶性病变及乳腺癌分期的应用价值.方法 回顾性选取2022年7月至2023年7月在南阳市第二人民医院乳腺科住院治疗患者80例,均为女性,入院后均接受乳腺数字断层合成摄影和动态增强(DCE)-MRI检查,以病理结果为金标准,分析比较良恶性乳腺病变患者的乳腺数字断层合成摄影和DCE-MRI影像学特征,比较2种方法单一检测以及两者联合对乳腺病变良恶性病变以及术前T分期诊断准确率.结果 乳腺数字断层合成摄影在诊断乳腺良恶性病变的灵敏度、特异度、准确度分别为83％、85％、82％;使用DCE-MRI诊断乳腺癌的灵敏度、特异度、准确度分别为84％、90％、88％;使用乳腺数字断层合成摄影联合DCE-MRI诊断乳腺癌的灵敏度、特异度、准确度分别为95％、97％、96％.结论 乳腺数字断层合成摄影联合DCE-MRI对于乳腺良恶性病变诊断具有较好的价值,可提高乳腺病变诊断灵敏度、特异度和术前T分期诊断准确率.
Abstract
Objective To explore the application value of breast digital tomography and dynamic MRI in the diagnosis of benign and malignant breast diseases and the staging of breast cancer.Methods A retrospective selec-tion was conducted on 80 patients who were hospitalized in the Breast Department of the Second People's Hospital of Nanyang from July 2022 to July 2023,all of whom were females.After admission,they underwent digital computed to-mography and DCE-MRI examinations of the breast.The pathological results were used as the gold standard to ana-lyze and compare the imaging features of digital computed tomography and DCE-MRI in patients with benign and ma-lignant breast lesions,Compare the accuracy of two methods for single detection and their combination in diagnosing benign and malignant breast lesions,as well as preoperative T-stage diagnosis.Results The sensitivity,specificity,and accuracy of using breast digital tomography synthetic imaging in diagnosing benign and malignant breast lesions were 83％,85％,and 82％,respectively.The sensitivity,specificity,and accuracy of DCE-MRI in diagnosing breast cancer were 84％,90％,and 88％,respectively.The sensitivity,specificity,and accuracy of digital tomography com-bined with DCE-MRI in the diagnosis of breast cancer were 95％,97％,and 96％respectively.Conclusion The combination of breast digital tomography and DCE-MRI has good value in the diagnosis of benign and malignant breast lesions,which can improve the sensitivity,specificity,and preoperative T-stage diagnostic accuracy of breast le-sions.
