摘要

目的 探讨磁共振液体衰减反转恢复(FLAIR)结合弥散加权成像(DWI)序列对急性蛛网膜下腔出血(SAH)的诊断价值.方法 选择抚州市南城县总医院2021年5月至2023年5月收治的83例疑似急性SAH患者为试验对象.所有受试者均接受磁共振FLAIR、DWI序列检查.以手术检查结果为金标准,分析磁共振FLAIR结合DWI序列诊断急性SAH的效能,对不同位置急性SAH病灶的检出情况.结果 将手术病理检查结果视作金标准,磁共振FLAIR结合DWI序列诊断急性SAH的灵敏度、特异度及准确性分别为98%、95%、98%,均高于FLAIR序列的89%、67%、83%以及DWI序列的85%、62%、79%(均P<0.05).磁共振FLAIR结合DWI序列对额顶部、纵裂池、外脑颞枕部、外侧裂池、侧脑室、后颅窝池及小脑上池SAH病灶的检出率均高于FLAIR序列与DWI序列(均P<0.05).结论 磁共振FLAIR结合DWI序列诊断急性SAH的价值较高,值得推广应用.

Abstract

Objective To study the diagnostic value of magnetic resonance fluid attenuated inversionrecovery(FLAIR)and diffusion-weighted imaging(DWI)sequences in the diagnosis of acute subarachnoid hemorrhage(SAH).Methods Eighty-three patients with suspected acute SAH admitted to Nancheng Ceneral Hospital of Fuzhou from May 2021 to May 2023 were selected as the subjects.All subjects underwent magnetic resonance FLAIR and DWI sequence examination.Taking the results of surgical examination as the gold standard,the efficacy of magnetic resonance FLAIR and DWI sequences combined with the diagnosis of acute SAH was analyzed,and the detection of acute SAH lesions in different locations was analyzed.Results Taking the results of surgical pathological examination as the gold standard,the sensitivity,specificity,and accuracy of FLAIR and DWI sequence combined with diagnosis of acute SAH were 98%,95%,and 98%,respectively,which were higher than 89%,67%,and 83%of FLAIR sequence and 85%,62%,and 80%of DWI sequence(all P<0.05).The detection rate of SAH lesions in frontal-parietal,longitudinal fissure cistern,lateral fissure cistern,lateral ventricle,posterior fossa cistern,and superior cerebellar cistern with FLAIR and DWI sequences was higher than that by FLAIR and DWI sequences(all P<0.05).Conclusion The combination by FLAIR and DWI sequence has a high value in the diagnosis of acute SAH,which is worthy of popularization and application.
