经阴道彩色多普勒超声及宫腔镜对子宫内膜病变的诊断价值

目的 探究子宫内膜病变患者应用经阴道彩色多普勒超声(简称"超声")及宫腔镜的诊断价值.方法 选择我院68例均为2021年5月至2023年10月收治的疑似子宫内膜病变患者,所有患者均行宫腔镜及经阴道超声检查.将病理检查结果作为"金标准",分析病理检查结果、对比单项及联合检查在子宫内膜病变中的诊断结果及在子宫内膜病变中的诊断价值,并计算与病理结果的一致性.结果 病理学检查共检出子宫内膜病变阳性51例,阴性17例.联合检查子宫内膜癌检出率高于宫腔镜及经阴道超声检查,差异有统计学意义(P＜0.05),其他子宫内膜病变不同检查方法对比中,差异均无统计学意义(P＞0.05).相比于经阴道超声及宫腔镜单项检查,联合检查在诊断子宫内膜病变中应用价值较高,差异有统计学意义(P＜0.05).一致性检验显示,联合检查与"金标准"一致性良好(kappa值=0.96,P＜0.001),宫腔镜一致性尚可(kappa值=0.57,P＜0.001),而经阴道超声一致性不佳(kappa值=0.37,P=0.002).结论 子宫内膜病变在经阴道超声及宫腔镜联合检查下应用价值较高,并与病理结果存在较强的一致性,可有效提升子宫内膜病变检出率,对指导临床治疗提供有效依据,值得推广应用.
Abstract
Objective To explore the diagnostic value of transvaginal color Doppler ultrasound(ultrasound)and hysteroscopy in patients with endometrial lesions.Methods A total of 68 patients with suspected endometrial lesions admitted to our hospital from May 2021 to October 2023 were selected,and all patients underwent hysteroscopy and transvaginal ultrasound examination.Using the pathological examination results as the"gold standard",analyze the pathological examination results,compare the diagnostic results of single and combined examinations in endometrial lesions,evaluate their diagnostic value in endometrial lesions,and calculate the consistency with the pathological re-sults.Results A total of 51 cases with endometrial lesions were detected as positive in pathological examination.Seventeen cases were negative.The detection rate of endometrial cancer in the joint examination was higher than that in the hysteroscopy and transvaginal ultrasound examination,with statistical differences(P＜0.05).There was no statis-tical difference in the comparison of different examination methods for other endometrial lesions(P＞0.05).Compared to single-item examination using transvaginal ultrasound and hysteroscopy,the combined examination has a higher ap-plication value in diagnosing endometrial lesions,with statistical differences(P＜0.05).The consistency test showed that the combined examination had good consistency with the"gold standard"(kappa value=0.96,P＜0.00l),and the consistency with hysteroscopy was still acceptable(kappa value=0.57,P＜0.001),while the consistency with transvagi-nal ultrasound was poor(kappa value=0.37,P=0.002).Conclusion Endometrial lesions have high application value under the combined examination of transvaginal ultrasound and hysteroscopy,and there is strong consistency with pathological results,which can effectively improve the detection rate of endometrial lesions and provide an effective basis for guiding clinical treatment.It is worth promoting and applying.
