摘要

目的 探讨CT颅脑灌注与磁共振成像(MRI)三维伪连续动脉自旋标记成像在颈动脉内膜切除术前、术后脑血流灌注改变一致性的研究.方法 选取我院2021年6月至2023年6月行颈动脉内膜切除术的颈动脉狭窄患者90例,术前及术后均经CT颅脑灌注与MRI三维伪连续动脉自旋标记成像检查,比较CT颅脑灌注与MRI三维伪连续动脉自旋标记成像手术前后参数值、相关性分析.结果 术前术侧脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)值低于对侧(P<0.05),术后两侧CBF、CBV、MTT值差异无统计学意义(P>0.05),与术前相比,术后术侧与对侧CBF值均升高,术侧升高程度大于对侧;而CBV、MTT均降低,术侧降低程度小于对侧(P<0.05);术前术侧脑血流量值低于对侧(P<0.05),术后差异无统计学意义(P>0.05),术后术侧与对侧脑血流量值均升高,术侧升高程度大于对侧(P<0.05),与CT颅脑灌注结果相似;基于CT颅脑灌注和MRI三维伪连续动脉自旋标记成像的CBF变化率相关性显示,术侧与对侧成正相关,且MRI三维伪连续动脉自旋标记成像的CBF变化率更高(P<0.05).结论 CT颅脑灌注与MRI三维伪连续动脉自旋标记成像均可测量颈动脉内膜切除术前后脑血流灌注情况,MRI三维伪连续动脉自旋标记成像在观察脑血流值变化上与CT颅脑灌注具有一致性且其为无创性,可用于定量评估手术前后脑血流动力学特征.

Abstract

Objective To investigate the consistency between CT cranial perfusion and MRI three-dimensional pseudo-continuous arterial spin labeling imaging in the changes of cerebral blood flow perfusion before and after carotid endarterectomy.Methods Ninety patients with carotid artery stenosis who underwent carotid endarterectomy in our hospital from January 2021 to June 2023 were selected.CT cranial perfusion and MRI three-dimensional pseu-do-continuous arterial spin labeling imaging were performed before and after surgery.The parameter values and corre-lation analysis before and after surgery were compared between CT cranial perfusion and MRI three-dimensional pseu-do-continuous arterial spin labeling imaging.Results The preoperative cerebral blood flow(CBF),cerebral blood vol-ume(CBV),and mean transit time(MTT)values on the surgical side were lower than those on the contralateral side(P<0.05),and there was no statistically signifiicant difference in CBF,CBV,and MTT values between the two sides af-ter surgery(P>0.05).Compared with preoperative values,the postoperative CBF values on both sides increased,and the degree of elevation on the surgical side was greater than that on the contralateral side.Both CBV and MTT de-creased,and the degree of decrease on the surgical side was smaller than that on the contralateral side(P<0.05).The preoperative cerebral blood flow value on the surgical side was lower than that on the contralateral side(P<0.05),and there was no difference after surgery(P>0.05).The postoperative cerebral blood flow values on both sides increased,and the degree of increase on the surgical side was greater than that on the contralateral side(P<0.05),which was sim-ilar to the results of CT brain perfusion.The correlation between the change rate of CBF values based on CT cerebral perfusion and MRI three-dimensional pseudo continuous arterial spin labeling imaging showed a positive correlation between the surgical side and the contralateral side,and the CBF change rate of MRI three-dimensional pseudo con-tinuous arterial spin labeling imaging was higher(P<0.05).Conclusion Both CT cranial perfusion and MRI three-dimensional pseudo-continuous arterial spin labeling imaging can measure cerebral blood flow perfusion before and af-ter carotid endarterectomy.MRI three-dimensional pseudo-continuous arterial spin labeling imaging is consistent with CT cranial perfusion in observing changes in cerebral blood flow values and is non-invasive,which can be used to quantitatively evaluate cerebral hemodynamic characteristics before and after surgery.
