摘要
目的 探究腹股沟疝患儿的高频超声表现及其诊断价值.方法 回顾性分析 2022 年 1 月至2023年 12 月我院收治的 63 例疑似腹股沟疝患儿的临床资料.所有患儿均接受高频超声和常规超声检查,以手术结果为标准,使用Kappa检验分析高频超声和常规超声检查结果与手术结果的一致性,并分析诊断效能.对比不同超声检查对直疝、斜疝(易复性疝、难复性疝、嵌顿疝及绞窄疝)的检出率.观察腹股沟疝的高频超声表现.结果 经手术结果确认,63 例患儿中共 56 例确诊为腹股沟疝,7 例为腹股沟区囊肿.常规超声检查结果与手术结果的Kappa值为 0.526,一致性一般;高频超声检查结果与手术结果的Kappa值为 0.662,一致性较高.常规超声检查对腹股沟疝诊断的灵敏度、特异度和准确度分别为 91%、71%、89%;高频超声检查对腹股沟疝诊断的灵敏度、特异度和准确度分别为 93%、86%、82%.高频超声对易复性疝的检出率高于常规超声(P<0.05),2 种超声对直疝、难复性疝、嵌顿疝及绞窄疝的检出率差异无统计学意义(P>0.05).高频超声下腹股沟疝肿物呈椭圆状或长条形,边界光滑、清晰.内部回声为液性或实性弱回声,检查过程中肿物大小可改变,向上可见疝囊通向腹腔.结论 高频超声检查可获得典型性的腹股沟疝肿物影像学图像,对患儿腹股沟疝的诊断价值较高,具有显著优势.
Abstract
Objective To explore the diagnostic value of high-frequency ultrasonography in children with inguinal hernia.Methods The clinical data of 63 children with suspected inguinal hernias admitted to our hospital from January 2022 to December 2023 were retrospectively analyzed.All patients received high-frequency ultrasound and conventional ultrasound examinations.Kappa test was used to analyze the consistency of high-frequency ultrasound and conventional ultrasound results with surgical results,and to analyze the diagnostic efficacy.To compare the detection rate of direct hernia,and oblique hernia(easy hernia,irreducible hernia,incarcerated hernia,and strangulated hernia)by different ultrasonography.The high-frequency ultrasonic manifestations of inguinal hernia were observed.Results A total of 56 cases of inguinal hernia and 7 cases of inguinal cyst were confirmed by operation.The Kappa value of routine ultrasound and surgical results was 0.526,which was generally consistent.The Kappa value between the results of high-frequency ultrasound and the results of surgery was 0.662,showing a high consistency.The sensitivity,specificity,and accuracy of conventional ultrasound in the diagnosis of inguinal hernia were 91%,71%,and 89%,respectively.The sensitivity,specificity and accuracy of high frequency ultrasound in the diagnosis of inguinal hernia were 93%,86%and 82%,respectively.The detection rate of easy hernia by high-frequency ultrasound was higher than that by conventional ultrasound(P<0.05),but there was no significant difference in the detection rate of direct hernia,irrecoverable hernia,incarcerated hernia,and strangulated hernia between the two kinds of ultrasound(P>0.05).Under high-frequency ultrasound,the inguinal hernia masses were elliptical or elongated,with smooth and clear boundaries.The internal echo is a liquid or solid weak echo,the size of the mass can change during the examina-tion,and the hernia sac can be seen upward to the abdominal cavity.Conclusion The imaging images of typical in-guinal hernia masses can be obtained by high-frequency ultrasonography,which has a significant advantage in the di-agnosis of inguinal hernia in children.
