摘要

目的 探究剪切波弹性成像技术、高频超声联合诊断在甲状腺结节定性诊断中的诊断价值.方法 回顾性收集 2022 年 2 月至 2023 年 8月于我院就诊的 160 例疑似甲状腺恶性结节患者临床资料,根据手术或病理活检结果可将其分为恶性组(24 例)、良性组(136 例),比较 2 组高频超声参数[阻力指数(RI)、搏动指数(PI)以及舒张期最低血流速度(D)]及剪切波弹性成像参数[弹性杨氏模量最大值(Emax)、最小值(Emin)、平均值(Emean)、剪切波速度及取样框内弹性数据离散度值(Esd)],比较不同检查方式的诊断结果及诊断效能.结果 恶性组剪切波速度、Emin、Emean、Emax、Esd值、D值、RI、PI值均高于良性组(P<0.05);高频超声联合剪切波弹性成像诊断的灵敏度为 91.7%,均高于单独检查方法(P<0.05),漏诊率为 8.3%,均低于单一诊断(P<0.05).结论 高频超声、剪切波弹性成像技术联合诊断在甲状腺结节性质鉴别诊断中可显著提高诊断灵敏度,降低漏诊率,可作为临床诊断甲状腺结节性质的辅助指标.

Abstract

Objective To explore the diagnostic value of shear wave elastography combined with high-frequency ultrasound in the qualitative diagnosis of thyroid nodules.Methods A total of 160 patients with suspected thyroid malignant nodules treated in our hospital from February 2022 to August 2023 were retrospectively collected,and they could be divided into a malignant group(24 cases)and benign group(136 cases)according to surgery or pathological biopsy.The two groups of high-frequency ultrasound parameters[resistance index(RI),pulsation index(PI),and minimum diastolic blood flow velocity(D)]and shear wave elastography parameters[maximum elastic Young's modulus(Emax),minimum elastic Young's modulus(Emin),average value(Emean),shear wave velocity and elastic data dispersion value(Esd)in sampling frame]were compared.The diagnostic results and diagnostic efficiency of different examination methods were compared.Results The shear wave velocity,Emin,Emean,Emax,Esd,D,RI,and PI values in the malignant group were significantly higher than those in the benign group(P<0.05).The diagnostic sensitivity of HF ultrasound combined with shear-wave elastography was 91.7%,both higher than that of single examination(P<0.05),and the missed diagnosis rate was 8.3%,both lower than that of single diagnosis(P<0.05).Conclusion The combined diagnosis of high-frequency ultrasound and shear wave elastic imaging can significantly improve the diagnostic sensitivity and reduce the rate of missed diagnosis in the differential diagnosis of thyroid nodule properties,and can be used as an auxiliary index for clinical diagnosis of thyroid nodule properties.
