The application value of antibiotics injected after alveolar lavage with bedside fiberoptic bronchoscopy in elderly patients with severe pneumonia
Objective To explore the application value of antibiotic injection after bronchofibroscope alveolar lavage in elderly patients with severe pneumonia.Methods The clinical data of 126 elderly patients with severe pneumonia admitted to our hospital from August 2021 to December 2022 were retrospectively analyzed.The patients were divided into an experimental group(n=57)and a control group(n=69)according to the treatment methods.Patients in both groups received bronchofibroscope alveolar lavage.The experiment group was treated with sensitive antibiotic injection into the lesion site after lavage.The levels of interleukin-6(IL-6),tumor necrosis factort-α(TNF-α),C-reactive protein(CRP),procalcitonin(PCT),PaO2,PaCO2,mechanical ventilation time,length of stay in ICU and clinical efficacy were compared before and after treatmen between the two groups.Results The total effective rate of the experimental group was significantly higher than that of the control group(P<0.05).After treatment,the levels of IL-6,TNF-α,CRP and PCT in the experimental group were significantly lower than those in the control group,while the levels of PaCO2 were significantly lower than the control group(P<0.05).The mechanical ventilation time and ICU hospitalization time in the experimental group were significantly shorter than those in the control group(P<0.05).Conclusions Antibiotics injection after alveolar lavage can reduce the levels of serum inflammatory factors,optimize the blood gas indicators and shorten the mechanical ventilation time and length of ICU stay.It can improve the overall efficacy in elderly patients with severe pneumonia.
