目的 探讨鼻内窥镜下低温等离子射频消融腺样体切除术对鼾症患儿睡眠质量及通气功能的影响.方法采用随机数字表法将我院收治的86 例鼾症患儿分为对照组与研究组各43 例,对照组采取传统扁桃体腺样体切除术,研究组采取鼻内窥镜下低温等离子辅助切除腺样体与扁桃体治疗.比较两组临床疗效、术后应激反应指标、通气功能及生活质量的差异.结果 与对照组比较,研究组手术时间、疼痛时间、症状缓解时间及住院时间均明显较短,术中出血量明显较少(P<0.05).对照组与研究组的总体有效率分别为90.7%、97.7%,差异无统计学意义(P>0.05).术后24 h,与对照组比较,研究组外周血中氧化应激指标丙二醛(MDA)、超氧化物歧化酶(SOD)、硝基酪氨酸(3-NT)水平显著较低(P<0.05),抗氧化应激指标谷胱甘肽(GSH)水平明显较高(P<0.05);与术前比较,两组患者术后MDA、3-NT水平明显降低(P<0.05),SOD、GSH水平明显升高(P<0.05).术后6 个月,研究组OSA-18 疾病特异性生活质量调查问卷总分低于对照组,睡眠障碍、身体症状、情绪不佳、白天功能、对监护人的影响等五个维度得分也显著低于对照组(P<0.05).结论 与传统扁桃体腺样体切除术相比,鼻内窥镜下低温等离子射频消融腺样体切除术能够有效清除腺样体,改善鼾症、提高生活质量.
The clinical effects of low-temperature plasma radiofrequency ablation adenoidectomy under nasal endoscope in children with snoring syndrome
Objective To investigate the effect of low temperature plasma radiofrequency ablation adenoidectomy under nasal endoscope on sleep quality and ventilation function in children with snoring syndrome.Methods Eighty-six children with snoring were randomly divided into a control group and a study group,43 in each group.The control group was treated with traditional tonsillectomy adenoidectomy.The study group was treated with nasal endoscope assisted low-temperature plasma ablation of adenoid and tonsils.The clinical effect,postoperative stress response indicators,ventilation function and quality of life were compared between the two groups.Results Compared with the control group,the study group had shorter surgical time,pain time,symptom relief time,and hospital stay,and less intraoperative bleeding.The overall effective rates of the control group and the study group were 90.7%and 97.7%,re-spectively.The difference was not statistically significant.After 24 hours postoperatively,compared with the control group,the levels of oxidative stress markers such as malondialdehyde(MDA),superoxide dismutase(SOD)and nitrotyrosine(3-NT)in the peripheral blood were significantly lower(P<0.05)in the study group.Conversely,the level of the antioxidant stress marker such as glutathione(GSH)was significantly higher(P<0.05).Compared with preoperative values,both groups showed a significant decrease in MDA and 3-NT levels(P<0.05),and a significant increase in SOD and GSH levels(P<0.05).After 6 months of surgery,the total score of OSA-18 as well as the five dimensions of sleep disorder,physical symptoms,bad mood,daytime function,and influence on guardians in the study group was significantly lower(P<0.05).Conclusions Compared with traditional adenoidectomy of tonsil,ade-noidectomy with low temperature plasma radiofrequency ablation under nasal endoscope can effectively remove the adenoid and improve the snoring symptom.As the results,the quality of life of children is improved.
