独活寄生汤内服联合中药外洗治疗类风湿关节炎肝肾亏虚证的疗效观察

摘要
目的 探讨独活寄生汤内服联合中药外洗治疗类风湿关节炎肝肾亏虚证的临床疗效.方法 选取2019年12月—2022年12月南京中医药大学徐州附属医院收治的86例类风湿关节炎肝肾亏虚证患者,依照随机数表法分为观察组和对照组各43例.观察组予甲氨蝶呤+硫酸羟氯喹,并配合独活寄生汤+中药外洗治疗,对照组予甲氨蝶呤+硫酸羟氯喹治疗.比较两组患者临床疗效、简化疾病活动指数(SDAI)、临床疾病活动指数(CDAI)、红细胞沉降率(ESR)、C反应蛋白(CRP)及白细胞介素-6(IL-6)等指标.结果 观察组总有效率高于对照组(P＜0.05).治疗后,两组CDAI评分和SDAI评分均较本组治疗前下降(P＜0.05),且观察组下降水平高于对照组(P＜0.05).治疗后,两组ESR、CRP、IL-6水平均较本组治疗前降低(P＜0.05),且观察组低于对照组(P＜0.05).结论 独活寄生汤内服联合中药外洗治疗类风湿关节炎肝肾亏虚证,改善患者临床症状,疗效确切,并且能够减少炎症因子释放,安全可靠.
Abstract
Objective To investigate the clinical effect of Duhuo Jisheng Decoction(独活寄生汤)orally combined with exter-nal washing of traditional Chinese medicine on rheumatoid arthritis with liver and kidney deficiency syndrome.Methods A total of 86 patients with rheumatoid arthritis syndrome of liver and kidney deficiency admitted to Xuzhou Affiliated Hospital of Nanjing U-niversity of Chinese Medicine,from December 2019 to December 2022 were selected and divided into observation group and con-trol group according to random number table method,with 43 cases in each group.The observation group was treated with Metho-trexate(MTX),Hydroxychloroquine(HCQ),Duhuo Jisheng Decoction and Traditional Chinese medicine external washing,and the control group was treated with Methotrexate and Hydroxychloroquine.Clinical efficacy,simplified disease activity index(SDAI),clinical disease activity index(CDAI),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and interleu-kin-6(IL-6)were compared between the two groups.Results The effective rate in the observation group was higher(P＜0.05).After treatment,CD AI score and SDAI score of two groups were decreased compared with that before treatment(P＜0.05),and the decrease level of observation group was higher than control group(P＜0.05).After treatment,the levels of ESR,CRP and IL-6 in both groups were lower than those before treatment(P＜0.05),and those in the observation group were lower than those in the control group(P＜0.05).Conclusion Duhuo Jisheng Decoction combined with traditional Chinese medicine for external washing in the treatment of rheumatoid arthritis patients with liver and kidney deficiency syndrome can improve clinical symptoms,effective,reduce the release of inflammatory factors,safe and reliable.
