中药有效成分通过调控miRNA的表达治疗糖尿病肾病机制的研究进展

摘要
糖尿病肾病(diabetic nephropathy,DN)是糖尿病最严重的微血管并发症之一,目前DN的一线药物主要是肾素-血管紧张素系统(RAS)抑制剂或血管紧张素受体阻滞剂等,但效果仍不理想,探索切实可行的DN药物仍是医疗上的难点和重点.有研究表明,在DN的发生和发展过程中,几个因子单独或协同作用以介导肾脏病理的基因表达.其中微小RNA(microRNA,miRNA)作为基因表达的调控因子,可通过作用不同信号通路独立或交叉地影响DN的发生发展机制.近年来大量研究发现,中药有效成分能够通过调控miRNA的表达,减轻肾小球系膜细胞、肾小球足细胞以及肾小管的损伤,达到治疗DN的作用.故通过对近年中药调控miRNA治疗DN的作用机制进行综述,旨在为中医药治疗DN的基础研究及临床应用提供一条新途径.
Abstract
Diabetes nephropathy(DN)is one of the most serious microvascular complications of diabetes.At present,the first-line drugs for DN are mainly renin angiotensin system(RAS)inhibitors or angiotensin receptor blockers,but the effect is still not ideal.Exploring practical DN drugs is still a difficult and key point in medical treatment.Some studies have shown that in the process of the occurrence and development of DN,several factors act alone or jointly to mediate the gene expression of renal pathology.Among them,microRNA(miRNA),as the regulatory factor of gene expression,can affect the occurrence and develop-ment mechanism of DN independently or crosswise by acting on different signal pathways.In recent years,a large number of stud-ies have found that the effective ingredients of traditional Chinese medicine can reduce the damage of glomerular mesangial cells,glomerular podocytes and renal tubules by regulating the expression of miRNA,and achieve the effect of treating DN.Therefore,by reviewing the mechanism of regulating miRNA treatment of DN by traditional Chinese medicine in recent years,it aims to provide a new way for the basic research and clinical application of traditional Chinese medicine treatment of DN.
