摘要
肠易激综合征(Irritable Bowel Syndrome,IBS)是一种常见的消化系统疾病,随着人们精神压力增加、饮食结构变化等,肠易激综合征在中国患病率逐渐升高,其中以腹泻型肠易激综合征(Irritable bowel syndrome with diarrhea,IBS-D)最为多见.目前现代医学治疗IBS-D虽短期疗效尚可,但仍有长期疗效欠佳、复发率高等缺陷,研究发现中医药治疗IBS-D有独特优势.参苓白术散作为治疗脾虚湿盛证IBS-D代表方之一,有益气健脾、运脾化湿的功效,现代药理学研究显示参苓白术散有调节肠道菌群、调控神经系统、修复肠屏障功能、抗炎、提高免疫等作用,可有效改善肠易激综合征胃肠道症状.该文对参苓白术散相关基础实验及临床试验进展进行综述,分析参苓白术散作用机制和临床应用,以期为提高临床疗效提供更多思路.
Abstract
Irritable Bowel Syndrome(IBS)is a common digestive system disease.With the increase of mental stress and the change of dietary structure,the prevalence of IBS in China is gradually increasing,and diarrhea predominant irritable bowel syn-drome(IBS-D)is the most common type.At present,although the short-term efficacy of modern medicine in the treatment of IBS-D is acceptable,it has the defects of poor long-term efficacy and high recurrence rate.Studies have found that traditional Chinese medicine has unique advantages in the treatment of IBS-D.Shenling-baizhu Powder,as one of the representative pre-scriptions for the treatment of IBS-D with spleen deficiency and dampness,has the effect of beneficial qi and spleen strengthe-ning,promoting spleen and removing dampness.Modern pharmacological studies have shown that Shenlingbaizhu powder can ef-fectively improve the gastrointestinal symptoms of irritable bowel syndrome by regulating intestinal flora,regulating nervous sys-tem,repairing intestinal barrier function,anti-inflammation,and improving immunity.This article reviews the basic and clini-cal experiments of Shenlingbaizhu powder,and analyzes the mechanism and clinical application of Shenlingbaizhu powder,in or-der to provide more ideas for improving clinical efficacy.
