从NF-κB信号通路探讨中药单体对糖尿病周围神经病变的保护作用

糖尿病周围神经病变(Diabetic peripheral neuropathy,DPN)是糖尿病最常见的慢性并发症之一,是糖尿病足溃疡、非创伤性截肢的主要原因.DPN以轴突变性坏死、神经纤维节段性脱髓鞘或髓鞘增生、雪旺细胞损伤、神经束膜增厚等神经损伤为主要病理特点[1].因此针对神经损伤进行治疗有利于延缓DPN的疾病进展.胰岛素抵抗、炎性反应、氧化应激等因素是其常见发病机制,由于发病机制暂未完全明确,尚没有具有针对此病的特效药.随着信号通路研究的不断深入,核因子-κB(Nuclear factor kappa-B,NF-κB)作为经典的炎性信号通路,其激活可以影响炎性反应、氧化应激、细胞凋亡、晚期糖基化终产物(Advanced glycation end products,AGEs)等DPN发病机制的多个方面,因此调控NF-κB信号通路对于DPN的治疗尤为关键.近年来,随着中药的深入开发,中药单体的研究受到广泛关注,诸多研究表明多种中药单体都可以通过NF-κB信号通路发挥治疗DPN的重要作用,该文通过对NF-κB信号通路的作用机制及有关该通路的中药单体研究进行了综述,以期为DPN的防治提供新的思路.
To explore the Protective Effect of TCM Monomer on Diabetic Peripheral Neuropathy From the NF-κB Signaling Pathway
Diabetic peripheral neuropathy(Diabetic peripheral neuropathy,DPN)is one of the most common chronic co-mplications of diabetes mellitus,and it is the main cause of diabetic foot ulcers and non-traumatic amputation.DPN features axonal degeneration and necrosis,segmental demyelination or myelin nerve hyperplasia,Schwann cell injury,and nerve bundle membrane thickening.Therefore,targeting nerve injury is beneficial to delay the disease progression of DPN.Insulin resistance,inflammatory response,oxidative stress and other factors are the common pathogenesis,because the pathogenesis is not fully clear,there is no specific drug for this disease.With the deepening of signaling pathway research,nuclear factor-κB(Nuclear factor kappa-B,NF-κB)as a classic inflammatory signaling pathway,its activation can affect inflammatory response,oxida-tive stress,apoptosis cells,late glycation end products(Advanced glycation end products,AGEs)and other aspects of DPN pathogenesis,so the regulation of NF-κB signaling pathway is particularly critical for the treatment of DPN.In recent years,with the further development of traditional Chinese medicine,the study of Chinese medicine monomer has attracted wide atten-tion,many studies show that a variety of Chinese medicine monomer can play an important role through NF-κB signaling path-way,through the mechanism of NF-κB signaling pathway and the pathway of TCM monomer research is reviewed,in order to provide new ideas for the prevention and treatment of DPN.
