Progress of research on the pathogenesis and pharmacological treatment of gastroesopha-geal reflux disease in Chinese and Western medicine
Gastroesophageal reflux disease(GERD)refers to the traumatic inflammatory disease of the esophageal mucosa caused by the reflux of stomach contents into the esophagus,with a complex pathogenesis in both Chinese and Western medicine,and a lack of systematic elaboration of the mechanism of drug action.Through searching relevant studies at home and abroad in re-cent years,we summarized the pathogenesis of GERD and the mechanism of drug action from both Chinese and Western medicine perspectives,and found that this disease is based on the basic pathogenesis of gastric Qi upward reversal,and that both the classi-cal prescriptions and new formulas,as well as single-flavored traditional Chinese medicines,reflect multi-targets,multi-path-ways to intervene in the transcriptional expression of related factors,inhibit the expression of pro-inflammatory genes and their receptors,down-regulate inflammation,inhibit gastric acid secretion,and enhance esophageal reflux disease(EGD).They in-hibit the expression of pro-inflammatory genes and their receptors,down-regulate the inflammatory response,inhibit the secre-tion of gastric acid and pepsin,and enhance the barrier function of the esophageal epithelium to promote the recovery of mucosal injury.Western medicine is often associated with mechanisms such as cytokine-mediated injury,and treatment is based on re-ducing gastric acid secretion,with a single mode and mechanism of action;therefore,Chinese medicine treatment has unique ad-vantages.In this paper,we would like to review the etiology,pathogenesis and drug treatment of GERD in Chinese and Western medicine,and construct the link between its pathogenesis and mechanism of action,so as to provide new ideas for the diagnosis and treatment of GERD in Chinese and Western medicine,as well as for the development of drugs.
