中医药治疗肾性水肿的研究进展

摘要
肾性水肿属中医学"水肿"等范畴,中医学认为其病因病机为脾肾亏虚,水湿内停,瘀水互结,治应温肾健脾,活血化瘀,行气利水.西医治疗肾性水肿以利尿为主,中医运用整体观念,以辨证论治的方法,重视机体自身,强化脏腑对水液的调节能力,其在消除临床症状、减轻西药不良反应以及改善预后等方面都具有一定的优势.现代药理研究证实中药主要通过抑制免疫炎症反应、调节蛋白通路、调节水电解质平衡、减轻肾小球屏障损伤、调节肠道菌群等多种途径治疗肾性水肿,具有多靶点、多途径的优势,且安全有效.文章对肾性水肿的中西医发病机制及中医药的应用现状、作用机理进行综述,为日后开展相关研究提供参考.
Abstract
Objective Renal edema belongs to the category of"edema"in traditional Chinese medicine,and its onset has the characteristics of lingering and difficult to heal,and traditional Chinese medicine believes that its etiology and pathogenesis are spleen and kidney deficiency,water and dampness are stopped,and stasis is intertwined.Western medicine is mainly diuretic in the treatment of renal edema,and traditional Chinese medicine uses a holistic concept,a method of syndrome differentiation and treatment,attaches importance to the body itself,and strengthens the ability of the viscera to regulate water and fluid,which has certain advantages in eliminating clinical symptoms,reducing adverse reactions of Western medicine and improving prognosis.Modern pharmacological studies have confirmed that traditional Chinese medicine mainly treats renal edema and protects the kid-neys by inhibiting immune inflammatory responses,regulating protein pathways,regulating water and electrolyte balance,allevia-ting glomerular barrier damage,regulating intestinal flora,etc.,which has the advantages of multiple targets and pathways,and is safe and effective.This article reviews the pathogenesis of nephrogenic edema in traditional Chinese and Western medicine,as well as the application status and mechanism of traditional Chinese medicine,so as to provide reference for future research.
