益气活血法靶向髓鞘脱失治疗中枢神经系统变性病的作用及机制研究

目的 中枢神经系统(CNS)变性病病因复杂，治疗困难，多途径、多靶点的中医药治疗效果良好。该研究旨在探究益气活血法靶向髓鞘脱失治疗CNS变性病的作用机制。方法 通过查阅近几年有关CNS变性病用药规律的文献，用IBM SPSS Statistics 20对其频次较高的单味中药进行统计学分析，对高频次药物在CNS变性病中的应用及其临床治疗效果和机制进行探讨。结果 通过数据分析，用药频次较高的药物共35味，累计频率160次，其中益气类占34％、活血类占32％;益气活血方治疗阿尔兹海默症(AD)/帕金森(PD)/多发性硬化(MS)/视神经脊髓炎(NMO)的临床疗效较好，治疗后患者生活质量提高，神经缺损程度改善，复发率降低;其治疗机制可能与减轻氧化应激、炎性反应、抗神经细胞凋亡从而抑制髓鞘脱失以及促进髓鞘修复有关。结论 益气活血法靶向髓鞘脱失是治疗CNS变性病的有效策略。
The therapeutic effect and mechanism of supplementing qi and activating blood circula-tion on neurodegenerative diseases by targeting nervous demyelination
Objective The etiology of central nervous system degeneration is complex,and the treatment is difficult.The thera-peutic effect of traditional Chinese medicine with multiple pathways and targets is good.The aim of this study is to explore the mechanism targeting myelination with the method of supplementing Qi and promoting blood circulation for central nervous system degeneration.Methods By reviewing literature on the medication patterns of central nervous system degeneration in recent years,IBM SPS Statistics 20 was used to conduct statistical analysis on single traditional Chinese medicine with higher frequency.The application of high-frequency drugs in central nervous system degeneration,as well as their clinical therapeutic effects and mech-anisms,were reviewed.Results Through data analysis,a total of 35 drugs with a higher frequency of use were found,with a cu-mulative frequency of 160 times.Among them,drugs of tonifying Qi accounted for 34％and drugs of activating blood accounted for 32％.The clinical efficacy of Yiqi Huoxue Fang in treating AD/PD/MS/NMO is good.After treatment,the quality of life of patients is improved,the degree of nerve defect is improved,and the recurrence rate is reduced.The therapeutic mechanism may be related to reducing oxidative stress,inflammatory reactions,inhibiting neuronal apoptosis,inhibiting myelin loss,and promo-ting myelin repair.Conclusion Targeting myelin sheath loss with the method of supplementing Qi and promoting blood circulation is an effective strategy for the treatment of central nervous system degeneration.
