中医药对男性不育患者疗效的Meta分析

摘要
目的 运用Meta分析观察中医药对男性不育患者的治疗效果,并进行相应的评价.方法 使用计算机检索重庆维普中文科技期刊数据库(VIP)、中国知网(CNKI)、中国学术期刊数据库(万方数据)、Pubmed、EMbase、Cochrane图书馆,检索从开始到结束的时间均为各数据库建库至2022年12月1日,搜集中医药与西药对比治疗男性不育的随机对照试验,由两位研究者按照纳入和排除的标准筛选文献,并将文献中的有关信息纳入.应用REVMAN5.4.1软件进行Meta分析.结果 共22篇治疗男性不育的文献被纳入.对1879例病例进行Meta分析结果表明,总有效率[RD=0.22,(95％CI:0.18～0.26),P＜0.01]、精子DNA碎片率[MD=-5.84,(95％CI:-6.99～-4.69),P＜0.01]、精子总活力[MD=5.90,(95％CI:3.68～8.15),P＜0.01]、精子浓度[MD=7.37,(95％CI:4.65～10.1),P＜0.01]、前向运动精子百分比[MD=7.50,(95％CI:5.91～9.08),P＜0.01].结论 中医药可明显降低患者精子DNA损伤,提高总有效率、精子总活力,并改善了精子浓度、前向运动精子百分比等指标,对男性不育症有良好的治疗效果.
Abstract
Objective To observe and evaluate the therapeutic effect of traditional Chinese medicine on male infertility patients by Meta analysis.Methods Chongqing VIP Chinese Science and Technology Journal Database(VIP),China National Knowledge Infrastructure(CNKI),China Academic Journal Database(Wanfang Data),Pubmed,EMbase,Cochrane Library were searched by computer.The time from the beginning to the end was from the establishment of each database to December 1,2022.Randomized controlled trials(RCTs)of traditional Chinese medicine and Western medicine in the treatment of male infertility were collected.Two researchers screened the literature according to the inclusion and exclusion criteria,and included the relevant information in the literature.Meta-analysis was performed using REVMAN5.4.1 software.Results A total of 22 articles on the treatment of male infertility were included.The results of meta-analysis of 1879 cases showed that.Total effective rate[RD=0.22,(95％CI:0.18～0.26),P＜0.01],sperm DNA fragmentation rate[MD=-5.84,(95％CI:-6.99～4.69),P＜0.01],total sperm motility[MD=5.90,(95％CI:3.68～8.15),P＜0.01],sperm concentration[MD=7.37,(95％CI:4.65～10.1),P＜0.01],percentage of forward motile sperm[MD=7.50,(95％CI:5.91～9.08),P＜0.01].Conclusion Traditional Chinese medicine can significantly reduce sperm DNA damage,improve total ef-fective rate and total sperm motility,and improve sperm concentration,forward movement sperm percentage and other indicators,which has a good therapeutic effect on male infertility.
