基于IGF-1/PI3K/AKT/mTOR信号通路的归脾汤对化疗肌少症的调节作用探讨

目的 探讨5-氟尿嘧啶(5-FU)诱导的肌少症的发生机制是否与胰岛素样生长因子-1(IGF-1)/磷脂酰肌醇-3-激酶(PI3K)/蛋白激酶B(AKT)/哺乳动物雷帕霉素靶蛋白(mTOR)信号通路有关,并用归脾汤进行调节。方法 选取40只SPF级雄性SD大鼠随机分为空白组、模型组(28mg/kg 5-FU)、归脾汤低剂量组(28mg/kg 5-FU+0。5g/ml归脾汤)、中剂量组(28mg/kg 5-FU+1g/ml归脾汤)、高剂量组(28mg/kg 5-FU+2g/ml归脾汤),每组8只,造模的同时归脾汤组灌胃不同剂量归脾汤,连续干预14天。末次给药后,大鼠禁水禁食,次日处死大鼠,取腓肠肌等以待检测,期间记录各组大鼠体重、抓力等生理状况。苏木素-伊红(HE)染色观察各组大鼠腓肠肌组织病理改变;免疫组织化学法(IHC)检测腓肠肌组织中肌萎缩相关蛋白Fbxo32及Trim63蛋白表达;酶联免疫吸附试验(ELISA)检测腓肠肌组织内胰岛素样生长因子-1(IGF-1)及胰岛素样生长因子-1受体(IGF-1R)的含量;实时荧光定量PCR(RT-PCR)测定腓肠肌组织中IGF-1、IGF-1R的mRNA表达水平;蛋白免疫印迹法(Western blot)检测腓肠肌组织中p-PI3K、PI3K、p-AKT、AKT、p-mTOR、mTOR的蛋白表达水平。结果 与空白组相比,模型组大鼠体质量、抓力均明显下降(P<0。01);肌细胞间隙增大、细胞核紊乱出现核内移,并可见炎性细胞浸润;IGF-1、IGF-1R蛋白及mRNA表达水平升高(P<0。01);Fbxo32、Trim63蛋白表达明显升高(P<0。01);p-PI3K/PI3K、p-AKT/AKT、p-mTOR/mTOR蛋白表达水平的比值显著降低(P<0。01)。与模型组相比,归脾汤低、中、高剂量大鼠体质量、抓力上升(P<0。05,P<0。01);腓肠肌内IGF-1、IGF-1R蛋白及mRNA表达水平升高(P<0。05,P<0。01);Fbxo32、Trim63 蛋白表达显著下降(P<0。05,P<0。01);p-PI3K/PI3K、p-AKT/AKT、p-mTOR/mTOR蛋白表达水平的比值显著升高(P<0。05,P<0。01)。结论 归脾汤对5-FU诱导的肌少症模型大鼠具有明显改善作用,可能与激活 IGF-1/PI3K/AKT/mTOR 信号通路,上调 IGF-1、IGF-1R、p-PI3K/PI3K、p-AKT/AKT、p-mTOR/mTOR 蛋白表达有关。
To explore the regulatory effect of Guipi Decoction on chemotherapy-related sarcopenia based on IGF-1/PI3K/Akt/mTOR signaling pathway
Objective To investigate whether the pathogenesis of 5-fluorouracil(5-FU)-induced sarcopenia is related to the insu-lin-like growth factor-1(IGF-1)/phosphatidylinositol-3-kinase(PI3K)/protein kinase B(Akt)/mammalian target of rapamycin(mTOR)signaling pathway,which is regulated by Guipi Decoction.Methods Forty SPF male SD rats were randomly divided into blank group,model group(28mg/kg5-FU),Guipi Decoction low-dose group(28mg/kg5-FU+0.5g/ml Guipi decoction),medium-dose group(28mg/kg5-FU+1g/ml Guipi decoction)and high-dose group(28mg/kg5-FU+2g/m)1 Guipi Decoction),8 animals in each group.At the same time,Guipi decoction group was given different doses of Guipi decoction,and the intervention lasted for 14 days.After the last administration,the rats were deprived of water and fasted,and the rats were killed the next day,and the gastrecne-mius muscles were taken for detection,during which the physiological status of the rats such as body weight and grasping power were re-corded.The pathological changes of gastrocnemius were observed by hematoxylin-eosin(HE)staining.The expression of muscular at-rophy related proteins Fbxo32 and Trim63 in gastrocnemius was detected by immunohistochemistry(IHC).The contents of insulin-like growth factor-1(IGF-1)and insulin-like growth factor-1 receptor(IGF-1R)in gastrocnemius were detected by enzyme-linked immunosorbent assay(ELISA).Real-time fluorescence quantitative PCR(RT-PCR)was used to determine the mRNA expression lev-els of IGF-1 and IGF-1R in gastrocnemius.Protein expression levels of p-PI3K,PI3K,p-Akt,Akt,p-mTOR and mTOR in gas-trocnemius were detected by Western blot.Results Compared with blank group,the body weight and grip strength of rats in model group were significantly decreased(P<0.01).The myocyte space was enlarged,the nuclei were disordered and the inflammatory cells were infiltrated.The expression levels of IGF-1 and IGF-1R protein and mRNA were increased(P<0.01);The expressions of Fbxo32 and Trim63 were significantly increased(P<0.01).The ratios of P-PI3K/PI3K,p-Akt/Akt,p-mtor/mTOR protein expression levels were significantly decreased(P<0.01).Compared with model group,the body weight and grip strength of rats with low,medium and high doses of Guipi decoction were increased(P<0.05,P<0.01).The protein and mRNA expression levels of IGF-1 and IGF-1R in gastrocnemius were increased(P<0.05,P<0.01);The expressions of Fbxo32 and Trim63 were significantly decreased(P<0.05,P<0.01).The ratios of P-PI3K/PI3K,p-Akt/Akt,P-mtor/mTOR protein expression levels were significantly increased(P<0.05,P<0.01).Conclusion Guipi Decoction has obvious improvement effect on 5-FU-induced sarcopenia model rats,which may be related to activation of IGF-1/PI3K/Akt/mTOR signaling pathway and up-regulation of IGF-1,IGF-1R,p-PI3K/PI3K,p-Akt/Akt,p-mTOR/mTOR protein expression.
